Extending the duration of clopidogrel treatment >12 months may decrease very late death or MI only in patients with DM receiving first-generation DES. Future studies should address this question in patients receiving second-generation DES.

Adequate platelet inhibition early in the course of ST-segment elevation myocardiai infarction treated by primary percutaneous coronary intervention is essential to reduce stent thrombosis and recurrent myocardiai infarction. Pharmacodynamic data now suggest that additional platelet inhibition beyond that provided by either prasugrel or ticagrelor might be necessary.
