Oro-antral Communication is an unnatural communication between the oral cavity and maxillary sinus and when it fails to close spontaneously, it remains patent and is epithelialized so that oro-antral fistula develops. It is a common occurrence following removal of maxillary premolars and molars because of anatomic proximity of root apices of these teeth and maxillary antrum. Signs and symptoms of oro-antral fistula varies from free escape of fluids, pain, pus leakage, voice alteration, to pan-sinusitis. Several surgical options exist for its management, in particular the buccal fat pad technique, which has proved to be an effective and a reliable technique. We report in this article two-succefull cases of oro-antral fistula managed with buccal fat pad.
The odontogenic keratocyst (OKC) is a benign intraosseous cyst of the odontogenic origin, characterized by an aggressive behavior with a relatively high recurrence rate.OKC is mostly presented at the 2nd and 3rd decades and predominantly affect males. It affects mandible more than maxilla and is frequently found at the angle of mandible. OKC can clinically and radiographically mimic other benign and less aggressive lesions. It is therefore important to differentiate them from other cysts and tumors, for a proper treatment and reduction of recurrences.We report the case of a 19-year-old woman who presented with a swelling in left mandible. The clinicaland radiographicfeatures were in favor of a radicular cyst.Histological examination, however, contradicted this diagnosis, indicating a case of OKC.
Multiple myeloma is a disease resulting from clonal proliferation of plasma cells. It is the most common primary bone malignancy. The most common clinical signs and symptoms of multiple myeloma include bone pain, fatigue, anaemia, infectious diseases and it is characterized by multiple punched‐out radiolucent lesions. Jaw lesions may be present in the advanced stages of multiple myeloma. Rarely, the oral and maxillofacial lesions can be the first manifestation of the disease. We report a rare case of a 55‐year‐old man who presented with a left mandibular swelling of a duration of 2 months. The panoramic radiograph showed a large destructive radiolucency in the left molar region and the angle of the left mandible. A biopsy was performed and revealed the diagnosis of a plasmocytoma. Further investigations of the patient revealed other osteolytic lesions in other bones, confirming the diagnosis of multiple myeloma.
Odontogenic sinusitis is a unilateral infection of the maxillary sinus caused by a dental origin. An ectopic tooth in the maxillary sinus is a rare situation that can cause sinusitis as well as dentigerous cysts. Diagnosis is based on a combination of clinical symptoms and radiographic findings. We report the case of a 40-year-old woman who presented with headache and hemifacial pain revealing a chronic sinusitis caused by a dentigerous cyst associated with ectopic third molar that was managed medically and surgically.
