A 29-year-old woman suffered a vesicouterine fistula following cesarean section. The fistula was treated successfully after cystoscopic identification and fulguration of the tract. Although various surgical approaches to this problem have been described, and a number of cases of spontaneous resolution are reported, to date there have been no reports of treatment of this problem via cystoscopic fulguration. We advocate this simple technique as a primary approach to vesicouterine fistulas.
Controlling COVID rates, which initially soared within New York State, proved to be more challenging for correctional institutions, which were tasked with trying to protect a captive population. This article profiles the Westchester County Department of Correction (WCDOC), a county jail located 20 miles north of Manhattan; it is the second-largest jail in New York State. By employing a multitude of modalities, the WCDOC was able to lower their COVID-19 rates significantly, while simultaneously providing medical, mental health, drug treatment, and courtrelated services. By leveraging the use of technology and employing innovative policies for prisoners and staff, the WCDOC can serve as a model for other correctional institutions.
We report our experience with 18 consecutive cases of priapism treated with intracorporeal irrigations of dilute epinephrine solution. Of the 18 patients 16 were treated successfully. The 2 failures had priapism 36 hours and 5 days in duration, respectively.
