Surgical treatment of chest wall deformities using this technique leads to good cosmetic, orthopedic and psychological results. We believe that the operations should be performed at any age in patients who have at least a moderate deformity.

Free Paper presented at 18th Congress of the Brazilian Academy of Neurosurgery ABNc RESUMO Apresentação do caso: Paciente com queixa de dispneia e cefaleia occipital à flexão do pescoço diagnosticada com luxação atlantoaxial secundária a AR. Encaminhada para artrodese cervical, sendo realizado posicionamento não usual e técnica cirúrgica de Harms. Discussão: Afecção de coluna cervical não é um achado incomum em pacientes com artrite reumatoide, mas que ainda apresenta subdiagnóstico importante, podendo cursar com compressão de estruturas e, assim, gerar os sintomas. Técnica de posicionamento visa uma melhor visualização intraoperatória dos parafusos, pois no momento de colocá-los é quando há o principal motivo de complicação da cirurgia, lesão de estruturas adjacentes. Técnica de Harms consiste em uma técnica de fixação C1-C2, sendo utilizado também no caso em questão de ampliação da fixação de C3 ao osso occipital. Comentários: Evidencia o sucesso de uma cirurgia que costuma ser evitada como tratamento, utilizando um posicionamento cirúrgico que melhora substancialmente a visibilidade dos parafusos pelo cirurgião, podendo diminuir consideravelmente as chances de possíveis complicações da cirurgia. ABSTRACTCase presentation: Patient complaining of dyspnea and occipital headache at flexion of the neck, diagnosed with atlanto-axial dislocation secondary to RA. Forwarded for cervical arthrodesis, being performed an unusual positioning and surgical technique of Harms. Discussion: Cervical spine affection is not an uncommon finding in patients with rheumatoid arthritis, but it still presents important underdiagnosis, being able to attend with compression of structures and thus generate the symptoms. Positioning technique aims a better intraoperative visualization of the screws, because their placement is the moment when occurs the main reason of complication of the surgery, injury of adjacent structures. Harms technique consists of a C1-C2 fixation technique, also being used in case of extension of the fixation of C3 to the occipital bone. Comments: It shows the success of a surgery that is usually avoided as a treatment, using a surgical positioning that substantially improves the visibility of the screws by the surgeon, and can considerably reduce the chances of possible complications of the surgery.
