Objective: Identifying Nursing Diagnoses of fatigue, activity intolerance and decreased cardiac output in hospitalized patients with heart failure and verifying the association between the defining characteristics and the Nursing Diagnoses. Method: A longitudinal and prospective study that followed hospitalized patients with heart failure for three weeks. The data collected through interviews and physical examinations were sent to expert nurses for diagnostic inference. Descriptive and inferential statistical analyses were carried out. Results: Of the 72 patients, 68.0% were male and presented the nursing diagnosis of decreased cardiac output (62.5%) in the first week, reducing to 52.8% and 38% in the second and third weeks, respectively. Fatigue only appeared in one patient. Activity intolerance was the diagnosis that had the greatest discrepancy among the experts. Decreased cardiac output was associated to the defining characteristics: dyspnea, edema, jugular venous distension and reduced ejection fraction during all three weeks of evaluation. Conclusion: Decreased cardiac output was more prevalent in hospitalized patients with heart failure, and the associated defining characteristics were determining factors for this nursing diagnosis.
Objective: To identify the NANDA International nursing diagnoses from the terms found in the nursing records of hospitalized children with congenital heart defects, and verify the association between these terms and the mapped nursing diagnoses. Methods: Observational and cross sectional study, developed by mapping of the terms in the nursing records of hospitalized children up to two years of age with congenital heart disease. The association between the terms and the most frequent nursing diagnoses were evaluated using the Student's t-test or chi-square. Results: The most frequent nursing diagnoses in the 82 records analyzed were: risk for infection (81.7%), impaired gas exchange (46.3%) and activity intolerance (36.6%). The terms "cyanotic" and "pallor" had significant associations with the diagnosis, impaired gas exchange. Conclusion:The terms recorded in the records of children with congenital heart disease allowed for the identification of NANDA International nursing diagnoses, in addition to verification of associations. ResumoObjetivo: Identificar Diagnósticos de Enfermagem da NANDA International a partir dos termos encontrados nos registros de Enfermagem de crianças com cardiopatias congênitas hospitalizadas e verificar associação entre estes termos e os Diagnósticos de Enfermagem mapeados. Métodos: Estudo observacional, transversal desenvolvido por mapeamento dos termos nos registros de Enfermagem de crianças hospitalizadas de até 2 anos com cardiopatia congênita. A associação entre os termos e os Diagnósticos de Enfermagem mais frequentes foi avaliada pelo teste t de Student ou qui quadrado. Resultados: Nos 82 registros analisados, os Diagnósticos de Enfermagem mais frequentes foram Risco de infecção (81,7%), Troca de gases prejudicada (46,3%) e Intolerância à atividade (36,6%). O termo "cianótico" e "hipocorado" tiveram relação estatisticamente significativa com o diagnóstico Troca de gases prejudicada. Conclusão: Observou-se que os termos registrados em prontuários de crianças com cardiopatias congênitas permitiram a identificação dos Diagnósticos de Enfermagem da NANDA International, além da verificação das associações.
RESUMO Objetivo: Analisar o efeito da entrevista motivacional no autocuidado de pessoas com insuficiência cardíaca crônica. Método: Ensaio clínico randomizado, multicêntrico, dividindo as pessoas em grupo intervenção (n=59) e grupo controle (n=59), acompanhadas por 60 dias nos centros do Brasil e Uruguai. O grupo intervenção recebeu três consultas por entrevista motivacional, com intervalo de 30 dias, e o grupo controle manteve o acompanhamento convencional nas clínicas especializadas. Os dados foram avaliados através do Self-Care of Heart Failure Index 6.2, antes e após a intervenção, em cada um dos centros e analisados através de média, mediana, teste-t, análise de correlação pelo coeficiente de Spearman e efeito da intervenção pelo d de Cohen. Resultados: 118 pessoas concluíram o estudo. Diante da avaliação do efeito da entrevista motivacional no autocuidado, comparando-se ao acompanhamento convencional, foi identificado efeito médio na manutenção e manejo (Cohen-d=0,6723; 0,5086) e alto na confiança do autocuidado (Cohen-d=0,9877). Conclusão: A entrevista motivacional foi efetiva na melhora do autocuidado dos pacientes com insuficiência cardíaca, sendo uma estratégia factível a ser implementada em clínicas especializadas. Objective:To evaluate the effect of the orientation group on therapeutic adherence and self-care among patients with chronic heart failure. Method:Randomized controlled trial with 27 patients with chronic heart failure. The intervention group received nursing consultations and participated in group meetings with the multi-professional team. The control group only received nursing consultations in a period of four months. Questionnaires validated for use in Brazil were applied in the beginning and in the end of the study to assess self-care outcomes and adherence to treatment. Categorical variables were expressed through frequency and percentage distributions and the continuous variables through mean and standard deviation. The comparison between the initial and final scores of the intervention and control groups was done through the Student’s t-test. Results:The mean adherence in the intervention group was 13.9 ± 3.6 before the study and 4.8 ± 2.3 after the study. In the control group it was 14.2 ± 3.4 before the study and 14.7 ± 3.5 after the study. The self-care confidence score was lower after the intervention (p=0.01). Conclusion:The orientation group does not improve adherence to treatment and self-care management and maintenance and it may reduce confidence in self-care. Registry REBEC RBR-7r9f2m.
Objective: to evaluate in the literature the effectiveness of the health education interventions in self-care and adherence to treatment of patients with Chronic Heart Failure. Method: a systematic review with meta-analysis. Studies were selected that compared health education interventions with the usual care to assess the outcomes of adherence and self-care. The quality of the methodological evidence was assessed by the Grading of Recommendations, Assessment, Development and Evaluation system. Results: the educational interventions were more effective in relation to the usual care in the outcome of adherence (fixed effect=0-3841; p-value <0.001). There was no statistical difference in the outcome of self-care (fixed effect=0.0063; p-value=0.898). Conclusion: the educational interventions improved the outcome of adherence, though not self-care in the patient with Heart Failure.
