A 2-year-old girl presented with jaundice of 2 weeks' duration. Ultrasonography of the abdomen revealed ascites and a dilated common bile duct suggestive of a choledochal cyst. Tc-99m mebrofenin hepatobiliary imaging demonstrated findings consistent with a diagnosis of spontaneous rupture of a choledochal cyst. Timely surgical intervention yielded excellent results. A follow-up hepatobiliary scan 4-months after surgery revealed no abnormality.
