The paper presents a cloud infrastructure for creation of interpretable diagnostic knowledge bases for intelligent decision support systems in medicine. An ontology of medical diagnostics allows us to form diseases regardless their etiology as a multivariate developing internal process. The relations types being defined in the ontology allow users to create a knowledge base sufficient for search of hypotheses of possible single diagnoses, for detection of combined and complicated pathologies, and for reduction of a set of hypotheses of diagnosis. The offered infrastructure includes the following tools: the knowledge base editor being generated on ontology, the terminology database editor, the tool for estimation of knowledge base quality and some instruments of automation of development and improvement of knowledge base. The infrastructure is implemented on the IACPaaS platform and used by experts to create knowledge bases in various fields of medicine. The article is useful for developers of medical diagnostics systems.
A review of modern software systems which solve different medical problems shows that there is a need for unified software-information environment which could provide access: for physicians to intelligent systems of decision support; for experts to knowledge bases and databases for their refinement and keeping in actual state; for students to simulators and training courses in different fields of medicine. Thus a concept of a cloud environment for development and use of a medical intelligent systems community is proposed. This environment concentrates medical information and software resources in a single virtual space and provides users with a wide access regardless of their geographical position. Information resources are ontologies (they define the structure of terms), knowledge bases and databases. Software resources are problem solvers (applied ones and system tools). Applied solvers are expert systems for decision support, case record editors, simulators, etc. System tools provide creation and management for information and software resources.

There was formed a new information resource for medical intelligent systems, which have been designed on IACPaaS cloud platform. The observations spread contains formal description of instrumental methods of patient diagnosis, which are most common in medical practice.
The paper considers a specialized cloud shell for development of medical expert systems and computer simulators. A distinguishing characteristic of the shell is the use of specialized ontologies to form knowledge bases independent of medical specialty.
