A pilomatrixoma is a benign skin tumor common in children, which develops from the matrix cells of hair follicles. It presents as a nodule or tumor of approximately 0.5-3 cm in size, with calcium-like consistency, faceted edges, and blue erythematous overlying skin. Sometimes, the skin may atrophy and look like a blister or even extrude calcium. Diagnosis is suspected based on the typical clinical presentation and supplementary imaging tests. The definite diagnosis is made according to histological findings. A diferencia de lo que ocurre en los adultos, suele pre- sentarse como una mácula, pápula, placa o nódulo normocoloreado, rojizo o rosado, sin cumplir con los criterios ABCDE habitualmente utilizados para diagnosticar esta entidad. Los diagnósticos diferenciales incluyen tanto patologías benignas como malignas. El diagnóstico debe confirmarse mediante el estudio histopatológico. El objetivo de este trabajo es comunicar el caso de una paciente de edad pediátrica con diagnóstico de melanoma en la que inicialmente se sospechó un nevo de Spitz y comparar las características del melanoma en los pacientes pediátricos con las observadas en los adultos.
