An investigative study was undertaken to determine the potential for a new magnetic resonance (MR) imaging technique, RODEO (rotating delivery of excitation off resonance), for use as a diagnostic imaging tool for the breast. The RODEO technique provides fat suppression with T1 weighting and is ideal for gadolinium-enhanced breast imaging. It is a short repetition time, steady-state sequence for high-resolution three-dimensional acquisitions and provides a clinically efficient imaging time of approximately 5 minutes for 128 sections. Imaging findings were correlated with serially sectioned pathologic specimens in 30 breasts with 47 malignant and 27 benign lesions. MR imaging had a sensitivity of 94% and a specificity of 37%. MR imaging depicted additional cancers not seen at mammography in 11 of the 30 patients (37%). The lesions not seen at mammography varied in size from 3 mm to 12 cm. RODEO MR imaging may be used to improve diagnosis of breast cancer in patients with mammographically dense breasts or silicone implants/injections and to stage disease in patients who are candidates for lumpectomy.
Simple epidermoid cysts of the testis are rare, benign lesions. The reported sonographic findings are varied, but to date there has been no report of an epidermoid cyst presenting as an echogenic mass with dense acoustic shadowing. We report such a case.
CASE REPORTA 52-year-old white man with prostatitis experienced vague, intermittent, right testicular pain. He had no prior trauma, swelling, redness, or weight loss. Physical examination revealed a discrete, rock-hard, nontender right testicular mass. The left testis was normal. No adenopathy was palpated. Real-time ultrasonography of the scrota1 contents using a 7.5-MHz transducer (Siemens Sonoline SE Erlangen, West Germany) revealed a well-circumscribed, 1.5-cm lesion within the parenchyma of the right testis. The mass had a highly echogenic margin with dense acoustic shadowing, characteristic of calcification ( Figure 1A). The right testis was not enlarged and was otherwise normal in texture. The left testis and both epididymides were normal. A radical right orchiectomy was performed for possible carcinoma.A nodular cystic mass measuring 1.5 cm in diameter was seen on cut section. The cyst contained yellowish, tan, cheese-like material, and had a calcific, fibrotic wall ( Figure 1B). Microscopic evaluation demonstrated a discrete cyst composed of collagenized fibrous tissue. No squamous epithelial lining was present, although the cyst contained squamous epithelial debris, heavy calcification, and ossification. No
