Introduction: An impacted tooth is a permanent tooth that has a completely developed root but which has failed to erupt. Many etiologies can lead to impaction, including a mechanical obstacle, such as benign tumor. Observation: An 11-year-old girl was referred for the avulsion of the left mandibular permanent canine, which was impacted. A radiolucent lesion with ground glass opacity was found on the eruption pathway of the tooth. Anterior radiographs showed the lesion that seemed to be the cause of the impacted canine. Anatomopathological examination revealed the lesion to be a juvenile trabecular ossifying fibroma. Comments: Ossifying fibromas are rare jaw tumors. They can be of various types: ossifying fibroma, psammomatoid, or trabecular juvenile ossifying fibroma. This is the first case that clearly shows the association between ossifying fibroma and tooth impaction. A literature review of ossifying fibromas and the management of fibro-osseous benign lesions of the jaws was performed. Conclusion: Although benign, juvenile trabecular ossifying fibroma can be locally aggressive and has a high rate of recurrence. It is very important to establish a precise diagnosis of a fibro-osseous benign lesion of the jaws to plan optimal management.
Introduction: Gorham-Stout disease is a rare idiopathic condition of the bone that is characterized by a massive and spontaneous osteolysis, with a vascular or lymphatic proliferation in bone, which is then replaced by fibrous tissue. Observation: a 16-year-old patient was referred to the maxillofacial surgery department to remove ectopic teeth bilaterally in the ramus. He had Gorham-Stout disease, managed for many years in orthopedic surgery department for a lower limb lesion and in neurosurgery department for a breach of the meninges. He was treated for 4 years with bisphosphonates. The removal of the ectopic teeth went well, with a simple postoperative course. Discussion: Gorham-Stout disease physiopathology remains unknown. Facial bones are often involved, especially the mandible. There are many possible treatments, but, due to the rarity of the disease, no therapeutic consensus exists. Bisphosphonates seem to be a good way to control this condition. So far, no case of bisphosphonates related osteonecrosis of the jaw has been reported in children. Conclusion: Gorham-Stout disease can involve the mandible and may lead to ectopic teeth.
Keywords: mucosal melanoma / sinus melanoma / pigmentation of the oral mucosa Abstract --Introduction: Mucosal melanoma (MM) is a rare malignancy of the head and neck. Sinonasal melanomas are the most frequent, followed by oral melanomas. Observation: A 67-year-old patient with a known left sinus melanoma initially treated with immunotherapy was referred for consultation concerning mobile teeth. A pigmented lesion infiltrating the left maxillary arch in the molar area was detected. The malignancy had invaded the oral mucosa. Despite many proposed treatments (immunotherapy, chemotherapy, and radiotherapy), the patient showed metastatic progression, which resulted in death. Discussion: Sinus MMs are malignancies with poor prognosis because these are often diagnosed in the late stages of disease progression. As these tumors are rare, there is no treatment consensus and surgery remains the best option. Diagnosis of pigmented lesions of the oral mucosa is sometimes complicated because of various implied etiologies. Conclusion: This case report shows that sinus MMs can induce pigmentation in the oral mucosa. These are rare malignancies with poor prognosis, for which no treatment consensus exists to date.
