Background and Aims:It is important to understand the anatomy of spinal cord and its surrounded organs to perform spinal anesthesia. And there are many factors relevant for this method. Herniated intervertebral discs, especially lumbar disc herniation, differ in degree, is one of common causes of low back pain. Onset may be at any age, most often between 25 and 50 yrs. In case we meet patients with this complication and other spine diseases including spinal canal stenosis, ossification or thickening of the posterior longitudinal or yellow ligament, we always take care not to provide cauda equina syndrome or other neurological risks. But limited information was revealed in previous reports. Therefore, in this study we observed the spinal canal and cauda equine using myerographic computed tomography to alert anesthesiologists about this common but potential status.Methods: 30 patients with diagnosis of spine degenerative diseases were evaluated. We observed their subarachnoid space, spinal canal and cauda equina using myerographic computed tomography.
Results:Depend on the degree of diseases and the level and the size of herniated nucreus, cauda equina showed various form adjacently in the limited subarachnoid space.Conclusion: Compressing the subarachnoid space or relative decrease of cerebrospinal fluid, degenerative changes of spine with spinal canal stenosis, ossification or thickening of the posterior longitudinal or yellow ligament and other kind of spondylosis may affect the level, success rate and the safety of spinal anesthesia. In this study, we showed interesting images and this may be useful to understand for and alert to anesthesiologist on performing centroneuraxis block. Myerographic computed tomography is a convenient technique that can show those vital neuraxial changes. 319.
