Bilateral dislocation of the shoulder is rare and still very rare when it arises during the sleep due to an epileptic seizure. We report in this work the case of a young 24-year-old patient not known epileptic before and who never has a convulsive crisis. We discuss the mechanism of this uncommon variety of shoulder dislocation and modalities of treatment.
Les fracture-luxations transolécraniennes sont une entité rare des fracture-luxations du coude. Il s'agit d'une lésion complexe qui peut compromettre le pronostic fonctionnel du coude. Dix patients étaient diagnostiqués dans notre service entre janvier 2005 et novembre 2012. Tous nos patients étaient de sexe masculin, l’âge moyen était de 29 ans. Les fractures de l'olécrane étaient complexes et comminutives dans sept cas et simples chez trois patients. Deux cas étaient associés à des fractures de la tête radiale (Mason III) et deux autres à des fractures de l'apophyse coronoïde. Les résultats étaient évalués après un recul moyen de trois ans et demi par le score de BROBERG et MORREY: trois cas étaient excellents, quatre bons, deux moyens et un mauvais. Cette lésion complexe nécessite une bonne reconstruction de la surface articulaire et une réparation des lésions associées pour permettre une rééducation précoce, seul garant d'une bonne récupération fonctionnelle.
The authors report eight cases of neglected dislocations of the elbow. The fracture-dislocations were included in our series. All patients benefited from a surgical reduction. In five cases, an internal fixation by K-wires was necessary. All patients benefited from physical therapy after removal of osteosynthesis material. After a mean follow-up of 20 months, we noted 50% of satisfactory functional results according to Mayo Elbow Performance Score. Neglected dislocations of elbow remain a rather frequent entity in developing countries. Its main cause is the negligence of patients and the recourse to traditional treatment. The prognosis of this injury remains reserved.
The osteogenic exostosis or osteochondroma usually reveals a tumor siting preferably in the knee area. Vascular complications exostoses are known and exceptional even if the frequency of these tumors is important, and we related a case report about a young man of 23 years old who suffers from a mass filling the popliteal area of the left knee, projecting on the popliteal pedicle. We proceed to the excision of the entire lesion. The arterial pseudoaneurysm is the most common vascular complication of exostosis especially in his popliteal location. MRI angiography is necessary, to mentioned osteochondroma aspect, the rule is: any painful exostosis must be removed because the risks of degeneration are real but rare.
