An 8-year-old girl presented with an 8-day history of a sharply demarcated, erythematous, tender pustular mass on the vertex of the scalp, associated with localized alopecia, scaling and tender cervical lymphadenopathy ( Fig. 1). Thus far, it had not responded to treatment with oral flucloxacillin. A few weeks previously, her twin brother had been commenced on topical terbinafine cream for presumed tinea corporis. His skin disease coincided with the familyÕs acquisition of two pet rats.
