Hospital X is one of type B hospital which is located in Mojokerto City. Based on data, the amount of medical toxic and hazardous waste at Hospital X was increasing during the covid-19 pandemic, in which this medical toxic and hazardous waste must be managed properly to prevent the occurrence of disease. The purpose of this study was to determine the medical toxic and hazardous waste management system at Hospital X Mojokerto Regency. The type of this research is qualitative descriptive research using form of interview sheets and observation sheets as instruments. The data obtained were primary and secondary data. The stages of data analysis in this research were data reduction, data presentation, and conclusion. The result of this research found that there were 2 stages of medical toxic and hazardous waste management at Hospital X Mojokerto Regency which weren’t in accordance with the Decree of Minister of Forestry and Environment Number 56 of 2015 and the Decree of the Minister of Health of the Republic of Indonesia Number 7 of 2019, that are transportation stage and storage stage. Abstrak: Rumah Sakit X merupakan salah satu rumah sakit tipe B yang berada di Kabupaten Mojokerto. Berdasarkan data, diketahui telah terjadi peningkatan jumlah limbah B3 medis di Rumah Sakit X selama masa pandemi Covid-19, dimana limbah B3 medis ini harus dikelola dengan baik untuk mencegah terjadinya penyakit. Tujuan dari penelitian ini yaitu untuk mengetahui sistem pengelolaan limbah B3 medis di Rumah Sakit X Kabupaten Mojokerto. Jenis penelitian ini merupakan jenis penelitian deskriptif kualitatif dengan menggunakan instrumen berupa kuesioner wawancara dan lembar observasi. Data yang diperoleh merupakan data primer dan data sekunder. Tahapan analisa data pada penelitian ini yaitu reduksi data, penyajian data dan penarikan kesimpulan. Hasil penelitian ini menemukan bahwa terdapat 2 tahapan pengelolaan limbah B3 Rumah Sakit X Kabupaten Mojokerto yang tidak sesuai dengan PERMEN LHK No. 56 Tahun 2015 dan Permenkes RI No. 2019, yaitu pada tahapan pengangkutan dan tahapan penyimpanan.
