Highlights COVID-19 patients with psychiatric disorders should be managed on a personalized basis. Risks of pharmacological interaction are not absolute and should be contextualized. Essential psychopharmacological ongoing treatments should be maintained. Most psychotropic drugs should be reduced 25–50% if given with lopinavir/ritonavir. Dose titration should be progressive and with ECG monitoring if cardiotoxic risk.
