Background: To evaluate the safety and efficacy of combined chemoradiotherapy or radiotherapy alone in elderly patients with esophageal carcinoma to identify the best method of treatment. Materials and Methods: One hundred and sixteen patients with esophageal carcinoma aged 70 and older who received definitive radiotherapy or chemoradiotherapy entered the study. Overall survival (OS), disease-free survival (DFS) and treatmentrelated toxicities were assessed. Results: The median OS of the overall population was 17.9 months. For patients treated with cCRT, sCRT and radiotherapy alone, the median OS was 22.3 months, 18.0 months and 12.4 months respectively(P=0.044). Median OS for patients treated with radiotherapy dose ≥60Gy and <60Gy was 20.2 months and 10.9 months respectively (p=0.017). By univariate analysis, Chemoradiotherapy (include cCRT and sCRT) and radiotherapy dose ≥60Gy were found to achieve higher survival rates compared with radiotherapy alone and radiotherapy dose <60Gy (P=0.015, P=0.017). By multivariate analysis, chemoradiotherapy (HR=1.645, P=0.022) and radiotherapy dose ≥60Gy (HR=1.642, P=0.025) were identified as independent prognostic factors of OS. Conclusions: Definitive concurrent chemoradiotherapy could be considered as a feasible and effective treatment in esophageal carcinoma patients aged 70 and older. Radiotherapy dose 60Gy is an effective treatment option compared with standard dose radiotherapy, while higher doses are not beneficial to improve survival.
Paper material has many excellent characteristics in architectural applications. Our main constructions for disaster rescue are temporary tents, board house and so on for a long time, so it is difficult to meet the psychological demands of the people in disaster area. According to Maslow's hierarchy theory of psychological demands, this paper analyzes people’s demands of physiology, psychology and social aspects in disaster areas, also sums up the exploration of paper material in architectural application, and finally summarizes that paper material is economy, flexible, safety, sustainable and so on.
Based on the smooth space thinking proposed by contemporary French philosopher Deleuze, this paper describes the ideas of architectural design from the mutual transition and conversion of buildings and environment, and the design of smooth space total three aspects, to provide the reference for future architectural design.
Combined with author's rich construction experience in water supply and sewerage works, analyzes and explores the background of energy-saving materials in water supply and sewerage works, dissects the situation of China's current energy-saving materials applied to the water supply and sewerage works, and forecasts the direction of the future development of new energy-saving materials.
Based on the idea of sustainable development, this paper thinks that the modular architecture is a combination of architectural thinking and architectural technology with distinct characteristics of times, which can make the current green building steadily advance on the road to industrialization. This paper also describes in detail about the current situation of the development of modular architecture in China from the two aspects of the enormous social, economic benefit and the development advantage position Combined with the examples, this paper further demonstrates the importance of modular construction in our country in the field of architectural industrialization under the idea of sustainable development.
