Objective: Penile torsion is a counterclockwise rotational anomaly of the penile shaft or glans. We aimed to evaluate the outcomes of penile degloving and dorsal dartos flap rotation used for the repair of isolated penil torsion. Material and methods:During evaluation of the patients admitted to our polyclinic for circumcision between January 2013-December 2014, 5 cases of isolated penile torsion were determined. Following the circumcision procedure performed with bipolar cautery, patients undergoing penile degloving were checked whether penile torsion was relieved or not. In case of insufficient improvement, torsion was corrected with application of dorsal dartos flap. Penile torsion was corrected with dartos flap in 2, and penile degloving in 3 cases. Results:The mean age of the patients was 5.6 years (4-7), and the mean operative time 12 minutes (7-20), respectively. The mean operation time was 17.5 (15-20) minutes and 8.3 (7-10) minutes in the dorsal dartos flap and penile degloving groups, respectively. The preoperative mean degree of penile torsion was 50° (30°-70°). The mean degree of torsion was 65° and 40° in the dorsal dartos and penile degloving groups, respectively. During the postoperative follow up, 1 case of residual torsion (<10°) was observed in the dorsal dartos flap group. Residual torsion was not observed in other patients. Conclusion:Exploration for isolated cases of penile torsion during the circumcision procedure should not be overlooked. Successful results can be obtained with penile degloving and dartos flap rotation in cases with low and moderate torsion.
Objective. Circumcision is practiced worldwide. Currently, the effect of age at the time of circumcision on wound healing is not fully known. This study aimed to determine the effect of age at the time of circumcision on wound healing. Methods. The study included 382 male patients aged ≤16 years that were circumcised between January 2014 and December 2014. Data for 345 patients that were followed-up regularly were evaluated retrospectively. Circumcision was performed using a bipolar diathermic knife. Circumcision wounds were considered healed when the 2 suture lines were observed to be completely apposed. Wound healing time was analyzed according to age at the time of circumcision. Results. Mean age of the patients was 7.2 years (range; 6 days-16 years). Mean duration of surgery was 327.5 sec and mean healing time was 4.3 days. The patients were classified according to age as group 1 (0-2 years; n=114; 32.9%), group 2 (3-6 years; n=60; 17.3%), and group 3 (7-16 years; n=171; 49.8%). Mean duration of surgery in groups 1-3 were 280.9 sec, 320.2 sec and 356.5 sec; respectively, and healing times were 4.1, 4.2 and 4.5 days; respectively. Healing time was significantly longer in group 3 than in groups 1 and 2 (p<0.05). There was no significant difference in healing time between groups 1 and 2 (p>0.05). Conclusion. Wound healing time was shorter in the patients aged 0-6 years than in those aged 7-16 years.Eur Res J 2016;2(3):206-210

Circumcision performed under local and/or sedation anesthesia. At the end of the circumcision, no patient was given any dressing and antibiotic treatment. Patients were called for check-up after the second day. Patients with full skin bonding between two sutures were accepted as having wound fully healed. The circumcision healing time and complication rates were evaluated. Cost analysis was completed. R Re es su ul lt ts s: : The average age of 623 patients was 6.7±5,3 years (6 days-16 years). The average operation duration and healing time of patients was 5.45±1,16 minutes and 4.3±0,8 days, respectively. Slight edema lasting 2-3 days was observed in 153 (24%) patients defined as minor complication. There was no major complication such as hemorrhage, burning and infection. The cost analysis found that each circumcision with bipolar diathermic blade had an average saving of 5.6 dollars.
