To overcome the disadvantages of other methods, the authors have devised a procedure for below-the-knee operations, involving a new technic of sciatic-femoral nerve block. The technic has so far been successful without use of adjuvant agents in 90 percent of over 100 patients so handled.
FIG. 2. Foot guard assembly for preventing inadvertent ventilation. A. Foot pressing pedal (not seen); B. Hinged baffle lifted by the foot in order to press pedal; C. Cover of foot guard; D. Switch.the foot-operated ARO valve was significantly less than with any of the hand-held devices tested. Therefore, this device is adequate for use with any of the currently-used systems that employ hand-held blowgun valves. The Whitey blowgun, which is admirably suited to the Sanders technic of ventilation for bronchoscopy,l or for microlaryngeal surgery,2 would be unsuitable for technics requiring jet flows greater than 100 L.lmin.3 One advantage of the foot pedal over handoperated systems is that the anesthesiologist has both hands free while he controls ventilation. This device has been used without complications on more than 50 patients. We recommend washing the foot-pedal switch with ether before fist putting the device into use, to remove any oil present. The manufacturers also provide dry-lubricated switches on request.
