A series of 76 patients with a primary pleomorphic salivary adenoma treated by simple extracapsular enucleation and high energy radiation between 1963 and 1976 is presented. Analysis of the results has shown a very low incidence of complications with only minimal cosmetic deformity. Of the 76 patients, only 5 have been lost to follow-up, 3 patients have died of causes unrelated to the tumour and only 1 patient has developed a recurrence. Further local excision in this patient showed the histological appearances of adenocarcinoma.
Summary.-A recently introduced assay of cell mediated immunity and humoral inhibitory factors has been evaluated in colorectal cancer patients. Using a perchloric acid extract of adenocarcinoma of the large bowel as antigen, 16/27 patients with colorectal cancer had significant cellular reactivity when their separated peripheral leucocytes were tested in homologous AB serum. In autologous serum only 7/27 had significant reactivity; 6/20 patients with a variety of other malignancies showed sensitization to the colorectal antigen preparation. It is concluded that the leucocyte adherence inhibition test may offer a simple method of assaying for serum blocking factors in sequential studies but will be of little value in the diagnosis of large bowel cancer. In vitro techniques for assessing im-human malignancies (Maluish anld Hallimunological reactivity have facilitated day, 1974). the study of the immune response to The principle of the test is that when human cancer and of factors which specifically sensitized lvmphocytes are modify its expression. The use of ultrasonic B-mode scanning of the liver and biliary tract in patients with persistent jaundice is described. Sixty-seven consecutive jaundiced patients have been scanned. A correct diagnosis was obtained in 66 per cent, useful information in 26 per cent and in only 8 per cent was the information misleading or of no diagnostic value. In our opinion this simple and non-invasive technique is of great value in the definite diagnosis of persistent jaundice and should allow earlier surgery in the obstructive cases.
The direct leucocyte migration inhibition technique has been extensively used in human tumour immunology and has the advantages of being rapid and relatively simple, requiring only a small amount of the patient's blood. It has the disadvantages of a lack of sensitivity and often difficult interpretation.
