El insulinoma es el tumor neuroendocrino del páncreas más frecuente. Se presenta el caso de un paciente de 54 años de edad. La sintomatología conocida por la disminución severa de la glicemia en el sistema nervioso central, el diagnostico se basó en la evidencia de hipoglicemia, asociado a niveles elevados de insulina. Los exámenes de imágenes contribuyeron a la localización del insulinoma. La intervención quirúrgica por laparotomía fue la enucleación.Se presentó recurrencia de las hipoglicemias. Recibió cursos de quimioterapia en forma irregular; a los 18 meses aproximadamente se produce el fallecimiento por complicaciones médicas y un cuadro de obstrucción intestinal.La Malignidad del tumor se estableció por criterios de inmunohistoquimica, tamaño de la tumoración, la evolución y seguimiento del caso evidencio la recurrencia e infiltración de la vena porta.Palabras clave: Insulinoma; Neoplasias Neuroendocrinas pancreáticas; Criterios de malignidad. The symptomatology known for the severe reduction of glycemia in the central nervous system, the diagnosis was based on evidence of hypoglycemia, associated with high levels of insulin. Imaging examinations contributed to the localization of insulinoma. Surgical ntervention was enucleation.Recurrence of hypoglycemia occurred. Received irregular chemotherapy courses; At approximately 18 months the patient died due to medical complications and intestinal obstruction.Malignancy of the tumor was established by immunohistochemical criteria, tumor size, evolution and followup of the case evidencing the recurrence and infiltration of the portal vein.
