Inflammatory fibroid polyps are uncommon; moreover, that polyp causing secondary intussusception in an adult are still rarer. Here, we report a case of inflammatory fibroid polyp of the small bowel that presented as just vague pain in the abdomen in a 48-year-old woman. Even though investigations reported Ileo-ileal intussusception caused by a polyp, the patient did not have clinical symptoms that could be correlated. The rareness of the disease made it a clinical challenge to subject the patient to laparotomy. The rareness of non-neoplastic condition being the cause for adult ileo-ileal intussusception and the clinical challenge associated with it makes it a case worth reporting.
Background: Abdominal wall hernia is the most commonly encountered clinical problem in the surgical setting and also the incidence of various types of hernias are known to have gender variations. This study was done to analyse the pattern of different types of hernia and their incidence in female patients.Methods: This was a retrospective study done at Basaveshwara Teaching and General Hospital, Kalaburagi, Karnataka, India from January 2012 to December 2014. All the female patients who presented with different types of hernia like inguinal, umbilical, ventral, congenital and diaphragmatic hernias irrespective of the age have been included in the study from January 2012 to December 2014.Results: In this study total of 87 cases of different types of hernias were studied including ventral, inguinal, umbilical and diaphragmatic hernias. Among which ventral hernias were the most common including 42 cases and among them one case presented as obstructed hernia and the most common age of incidence was 31-40 years. The next most common presentation was inguinal hernia with total of 29 cases and the paediatric age group (0-10 years) was the most common presentation. The next common hernia was umbilical hernia and the most common age of presentation was 21-40 years. The least common presentation was that of diaphragmatic hernia with only 4 cases, age of the patients being 60 and above.Conclusions: In present study, most commonly encountered hernia was ventral hernia followed by inguinal, then umbilical and lastly diaphragmatic hernias.
BACKGROUNDDiabetes is an ancient disease, epidemic and an economic burden for the present era. Diabetes and its longterm major complications include septic, vascular (either microvascular or macrovascular) and neuropathic lesions which affect the various anatomical regions of the body. Majority of admissions to surgical wards are due to these surgical complications of diabetes.The objectives are to study the surgical complications of diabetes, their outcome and management modalities aimed at studying age, sex, regional pattern, socioeconomic pattern, mode of presentation, type of lesions, various modalities of treatment available in the hospital, and outcome of each modalities practised in the hospital. MATERIALS AND METHODSThis is a descriptive study of 72 cases of diabetes mellitus with surgical complications like carbuncles, foot ulcers, neuropathy, etc. admitted in surgical wards of Basaveshwar Teaching and General Hospital, during the period extending from June 2015-May 2016. RESULTSIt was found that these complications usually occur in the age of 51-60 years. The ratio of male: female is 2.13:1. And men who worked outdoors like agriculturists were commonly affected with these complications. Nearly 58 patients in this series of 72 had past history of trauma and subsequently lesions developed. The common lesions are septic lesions of the foot in the form of ulcers, cellulitis and gangrene, followed by vascular and neuropathic lesions. Most of them were managed conservatively. 13 cases underwent incision & drainage and came for regular followup, 10 cases underwent disarticulation of toes. 4 patients underwent below-knee amputations, whereas 8 patients underwent above-knee amputations and 7 patients needed split skin grafting for covering of raw area. Mortality rate of this series of 72 patients was 4.16% (n=3). CONCLUSIONIt was concluded that the known male diabetics with trivial injuries, on irregular treatment, who work outdoors usually present with lesions requiring admission and intervention. Commonly affected site is lower limb followed by upper limb.
