Cysticercosis is an endemic disease in multiple areas of the world, caused by ingestion of embryonated eggs of Taenia solium. We reported a case of a 66-year old diabetic male presenting with decrease of consciousness and “rice grain” calcification in both hemithorax and neck soft tissue region, without findings of cysticercosis in computed tomography (CT) scan. Cysticercosis most commonly affects the central nervous system, causing neurocysticercosis (NCC), while solitary intramuscular cysticercosis without involvement of the central nervous system is rare. Treatment of cysticercosis is indicated for symptomatic patients, including NCC surgery is indicated in patients with neurovascular compromise caused by growth of cyst. Recognizing the signs and symptoms of cysticercosis is crucial for diagnosis and treatment in endemic areas where Taenia solium is prevalent.
Tuberculosis (TB) still becomes a significant health problem in developing countries, especially in Indonesia. Allergy to antitubercular drugs is a real hindrance to the management of TB. The first-line anti-TB drug (ATD) is still the most effective TB drug, but it can have some side effects. One of these side effects is drug hypersensitivity reactions (DHR) which can affect a patient's compliance. The recommended diagnostic approach to DHR is a graded challenge by introducing drugs safely and optimally under a threshold dose until the usual daily dose is reached. We present a case of DHR to ATB drug and oral graded challenge as the diagnosis approach.
The Charcot Joint Osteoarthropathy also referred as Charcot foot syndrome is a serious lower limb-threatening complication of diabetes. A patient was diagnosed with Charcot joint osteoarthropathy (COA) and found to have uncontrolled Diabetes Mellitus since more than 10 years. This case is crucial considering that Diabetes mellitus is a common case that we encounter, but the consequences of not being controlled can lead to serious complications. Accurate diagnosis is not always easy and can be a considerable clinical challenge. It needs to be differentiated from other conditions that has similar symptoms. Early diagnosis and initiation of treatment is crucial to avoid devastating and permanent complications.
