An 8-month-old female infant is brought to the emergency department due to the appearance of a mass through the vaginal introitus and blood spotting in the diaper over the last 2 days, with apparently maintained urine output and without any other symptoms. She was previously healthy, except for a history of acute pyelonephritis at 6 months of age (Escherichia coli). The examination showed a homogeneous exophytic lesion arising from the vaginal introitus, with a pearly violet colour, elastic consistency, friable, non-adherent to adjacent tissues, measuring 2 cm in diameter, along with an extensive diaper fungal dermatitis (Fig. 1). The rest of the examination was normal with an excellent general condition. The urinalysis showed haematuria, leukocyturia and nitrituria, and the blood analytical evaluation showed no significant alterations. The urine culture was negative.
