A 55-year-old postmenopausal woman with a history of 1 year of hyperandrogenism is presented. She had high plasma testosterone levels and signs of masculinization such as facial hirsutism, deepening of the voice, and temporal balding. A computed axial tomography scan of the adrenal glands was normal. No mass was detected by a bimanual pelvic examination or by pelvic ultrasonography. Following a bilateral oophorectomy, plasma testosterone levels fell within the normal range. Careful pathologic examination of the ovaries revealed a mall hilus cell tumor.
