Con la globalización y desde los años 90 existe un verdadero problema de las drogas. Tanto el consumo, la producción como la circulación de las mismas perturban al Estado en todos los ámbitos: económicos, sociales, políticos y educativos. La region andina, si bien solo se le apuntaba por ser productora, es ahora consumidora y las políticas públicas actuales no parecen ser adecuadas. Por ende Las drogas se deben estudiar sin tabú ni falsos conceptos acerca de ellas y aceptar la realidad para evitar un empeoramiento de la situación. ¿Por qué la prohibición de las drogas? ¿Son malas todas las drogas? ¿La culpa solo la tiene la Region Andina? ¿Pueden las grandes potencias intervenir en los territorios nacionales? La guerra contra las drogas y la prohibición son un fracaso en la region andina, por ello la legalización del mercado aparece entonces como única solución.
Rupture of the patellar tendon must be diagnosed urgently because reconstruction of the extensor mechanism produces better results when it is performed in acute conditions. Reconstruction of chronic extensor mechanism rupture on the contrary is very challenging. Several surgical techniques have been described using a variety of graft choices and fixation methods, but the optimal approach is still under debate. We report our experience of two cases of chronic patellar tendon rupture reconstruction using an Achilles tendon allograft reinforced by a vascularized ipsilateral semitendinosus tendon frame. The rapid functional recovery of the range of motion, only three months postoperatively, showed us that this reconstruction technique was effective.
Acute quadriceps tendon ruptures disrupt the continuity of the extensor mechanism and must be treated surgically. Tendon reattachment with suture anchors has recently been shown to be biomechanically superior to reattachment through transosseous tunnels. In this context, arthroscopy is an interesting alternative. It can limit postoperative wound healing and infectious complications, while ensuring functional outcomes that are equivalent to standard techniques. The goal of this technical note is to describe an original, reproducible method for arthroscopic reattachment of the quadriceps tendon with suture anchors. Four patients were operated using this technique in a pilot study. No wound healing, infectious complications or re-tears were observed. At 6 months' postoperative, the mean subjective IKDC score was 85.8/100.
