The prevalence of peripheral arterial disease requires the search for an optimal solution for preserving blood flow by using devices of various modifications. Drug-coated balloons are widely used in the surgical treatment of peripheral arterial disease. The article aims at the summarization of the study results on the efficacy and safety of the various drug-coated balloons, in particular, with paclitaxel, in the treatment of steno-occlusive lesions in the peripheral arterial disease. Comparing the study results is fraught with certain difficulties, given the differences in the study endpoints, the demographic characteristics of patients and the lesion patterns. Nevertheless, the study results of percutaneous transluminal angioplasty (PTA) with drug-eluting ballons on the femoropopliteal segment are superior to the results obtained using conventional PTA and other endovascular interventions in the same vascular bed. PTA with drug-eluting balloons has a Class 1 recommendation in accordance with the issued recommendations of the SCAI (Society for Cardiovascular Angiography and Interventions). However, conducting PTA with paclitaxel-eluting balloons requires further determination of the possibilities of its use. Introduction. The necessity to improve methods of adenomyosis treatment is determined not only by its high prevalence, which reaches 40 % among women of reproductive age, but also by its association with infertility of unclear genesis. The disease significantly reduces the quality of life.   Objective. To optimize reproductive outcomes after surgical treatment of nodular adenomyosis and to prevent the disease recurrence.   Materials and Methods. A cohort prospective comparative study examined the efficacy of therapy with dienogestam after laparoscopic removal of adenomyosis nodules with temporary uterine artery occlusion and temporary balloon occlusion of the internal iliac arteries. The study included 25 women who underwent organ-preserving treatment for nodular adenomyosis from January 1, 2020 to January 1, 2022. Inclusion criteria: age 22–45 years, mean 32 ± 4.5; organ preserving treatment for nodular adenomyosis, no contraindications to hormone therapy, informed voluntary consent of the patient to participate in the study.   Results. The use of Dientogest at a dose of 2 mg/day in a continuous regimen for 6 months after surgical treatment for nodular adenomyosis was shown to transform the intensity of pain syndrome to insignificant, the absence of recurrent disease for 24 months of follow-up.   Conclusions. The comprehensive treatment of nodular adenomyosis, which included laparoscopic removal of the nodule and the drug therapy with dienogestam, permitted the achievement of reproductive function in 40% of the patients. The choice of the treatment method for nodular adenomyosis depends primarily on the reproductive plans of the patient, the size and location of the adenomyosis nodules, and its long-term results depend on the comprehensive antiretroviral therapy.

Objectives: Remote endarterectomy of external and common iliac artery occlusions through a single, groin incision under fluoroscopic guidance is a relatively unknown surgical procedure. This prospective singlecenter cohort study describes this less invasive endovascular technique with the ring strip cutter and its early complications. The results at midterm follow-up are presented.Methods: From April 2004 to July 2010, 49 remote endarterectomies of the external or common iliac artery were performed in a retrograde manner from a single, groin incision in 48 patients (30 men, 31 procedures). The median age was 66 years (range, 39-82 years). Indications for operation were severe claudication in 28 (57%), rest pain in 13 (27%), and gangrene in 8 (16%) procedures. Follow-up included clinical evaluation, ankle-brachial index, and duplex scanning at 6 weeks, 3 months, and yearly thereafter. Results: Intraoperative technical success was achieved in 43 procedures (88%). A retroperitoneal incision was necessary in three patients for an additional arteriotomy in the iliac artery and in three others for a bypass procedure. The mean follow-up was 20 months (range 2-77 months). Three-year cumulative primary patency rate by means of lifetable analysis was 60.2% Ϯ 12.0% (SE). During follow-up, percutaneous transluminal balloon angioplasty with and without stenting was performed in six and two patients, respectively, resulting in a 3-year primaryassisted patency rate of 85.7% Ϯ 9.56%. Three-year secondary patency was 94.2% Ϯ 5.50%.Conclusions: Remote endarterectomy in external and common iliac arterial occlusive disease is a feasible endovascular procedure with a low complication rate. The midterm primary-assisted patency rate is good.
