Objective: To determine the usefulness of uterine artery Doppler in evaluation of third trimester high-risk pregnancies in prediction of adverse perinatal outcome. Methods: In this prospective study uterine artery Doppler parameters such as PI and early diastolic notch were recorded in a group of 60 high-risk pregnancies mainly consisting of pre-eclampsia and small for gestational age. Uterine artery score (UAS) was calculated from the Doppler parameters and a score > 2 was considered abnormal. Perinatal outcome was then correlated to this score and compared with umbilical artery Doppler. Results: Abnormal UAS was noted in 72% of the study group while umbilical artery Doppler was abnormal in only 35%. All the five perinatal deaths and high-rate of perinatal morbidity was observed when both uterine and umbilical artery Dopplers were abnormal. Perinatal morbidity parameters such as birth weight < 2 kg, gestational age < 34 weeks and NICU admission was significantly high in 23 women who had abnormal UAS with normal umbilical artery Doppler. In pregnancies beyond 34 weeks abnormal uterine artery Doppler was a better predictor of adverse outcome than umbilical artery Doppler. Conclusion: Abnormal uterine artery Doppler is a reliable predictor of adverse perinatal outcome in high-risk pregnancies with preeclampsia and fetal growth restriction. Normal uterine artery Doppler in third trimester is reassuring. Thus inclusion of uterine artery Doppler along with umbilical artery Doppler in high-risk pregnancy in third trimester will improve fetal surveillance.
Background: Number of pregnant women with preexisting diabetes is increasing. Hence the detection and management of diabetes from the beginning of pregnancy itself will help to improve the fetal and maternal outcome. Aim of the study was to determine the prevalence of pre-gestational diabetes among the antenatal women attending a tertiary care center and to study the associated factors. Methods: This was a cross sectional study conducted in SAT hospital, Government Medical College, Trivandrum, Kerala, India for one year. HbA1C was the test used to diagnose diabetes. 400 women attending the OP in their first trimester were selected after informed consent. Socio demographic factors assessed by a structured questionnaire. Blood samples were taken for HbA1C. Diagnosis of diabetes was made at levels of HbA1C ≥6.5%. Statistical tests used were mean, standard deviation, chi-square and odds ratio. Results: Prevalence of pre-gestational diabetes was 3.8%.in our study. Main associations were age more than 25 years, body mass index, family history of diabetes, history of intra uterine death, gestational diabetes in previous pregnancy, candidiasis and thyroid disease. Conclusions: Screening of all pregnant women in first trimester itself for diabetes will help in early detection of pregestational diabetes, so that anticipation of adverse outcomes and proper management can be done in such cases.
Comfort is a sense of both physical ,psychological aspect of individual . It may not be realistic to think that pregnant mothers . Providing proper position to mothers is one of the easiest method for maintain comfort. The objectives were to assess the maternal parameters among antenatal mothers before positioning, to assess the effect of different maternal positions on maternal parameters and fetal heart rate among antenatal mothers during non stress test, to compare the effect of different maternal position on maternal parameters and fetal heart rate among antenatal mothers during non stress test, to find out the association between the selected demographic variables with the maternal parameters and fetal heart rate. Experimental research design was used. The study was conducted in MGMCRI and St.Joseph Hospital ,cluney ,Puducherry. 60 samples selected based on the simple random technique. Result: The study findings revealed that, both maternal back pain and fatigue was reduced in semi fowlers position than left lateral position. It was statistically significant at p<0.05 level.
