Patient Present History:-On September 3rd, 2020, a 32-year-old male patient was admitted to hospital with a specific complaint of abdominal pain for the past three years. He began drinking alcohol on a daily basis. Initially, he only drank at night, but later he began drinking during the day. Clinical Finding: Mood irritable, social withdrawal, mental confusion, seizure, vomiting, mood changes, lethargy and vomiting. Diagnostic Evaluation: Hb - 11.9 mg%, MCV – 87 cum/micron, MCH- 28.9 pico/gm, Total Bilirubin- 1.47.USG –He is appearance was natural once more. There was no localized lesion or oblivious mass lesion in the pelvis.
BACKGROUND A broad spectrum of emotional lows, from simple sadness to a pathological suicidal condition is defined as a depression. This is a common mental illness that has been noticed in daily life. Usually, a person who is depressed or feeling unhappy may return to normal emotional health within a specified time. There are times where this condition stretches for such a time that it is greatly overshadowed by the value of the precipitating factor. The patient in a prolonged depression state withdraws from his / her private, social, and professional activities. In some scenarios the diagnosis of depression should be identified. According to epidemiological studies, 15 percent to 20 percent of the children and adolescents experience at least one depressive episode in life and this is a common recurrent and impairing condition that predicts future suicide attempts, academic failure, interpersonal problems, unemployment, substance abuse, and delinquency before adulthood. The objective of the study was to assess the prevalence of associated factors of depressive symptoms among school going children and to evaluate the association of the prevalence score with selected demographic variables. METHODS This was a cross sectional study with quantitative research approach. The study was conducted in a selected school of Wardha district. Nonprobability convenience sampling technique was used to collect data from school going children. A structured questionnaire including socio demographic details & patient health questionnaire (PHQ-9) scale was used to identify the prevalence of depressive symptoms among school going children. After collecting data, the sample characteristics were described by frequency and percentage. A chi-square test was used to find out the relationship between the association between prevalence of associated factors of depressive symptoms and selected demographic variables. RESULTS In this study 8.89 % of school going children had no depression; minimal depression was seen in 68.89 %; mild depression in 17.88 %, moderate depression in 4.44 %, and moderately severe depression in 0 % of school going children were seen. CONCLUSIONS There was a significant association between the prevalence of associated factors of depressive symptoms among school going children with gender only. Background: Congenital deformity of both feet is also called clubfoot. A common congenital orthopedic condition characterized by an excessively turned in the foot (equinovarus) and high medial longitudinal arch (cavus). If neglected the deformity can result in long‐term disability and pain. Interventions can be conservative such as splinting or stretching or surgical. Talipes equinovarus is one of the more common congenital abnormalities affecting the lower limb and can be challenging to manage. There is evidence for a genetic contribution to congenital talipes equinovarus etiology. Case Presentation: A 7 years old Male child came to the Pediatric department with a chief complaint of deformity of both feet since birth. No Interventions were done during the first month of the baby. And that serial casting was done on both feet for 1 year, yet no correction was observed. After 1 year of age posterior soft tissue release of the right leg. After every 10 days, serial casting was done and yet no correction was observed. Now came to the Pediatric department for further management. On arrival, a physical examination of the foot was carried out which shows swelling over the right leg along with tenderness. The Contrast Enhanced Computed Tomography was done later he was treated with antibiotics, analgesics, and antacids. Conclusion: It is a congenital deformity foot. The congenital deformity of the foot may occur lateral and bilateral. The congenital deformity of the bilateral foot is common in children. The bilateral deformity of the foot affects 80% or more. The most significant contributing component to its cause appears to be heredity. The prevalence of congenital talipes is uncommon and roughly equal in both sexes.
Introduction: Gastric cancer or stomach cancer is an any malignant tumor arising from the region extending between the gastroesophageal (GE) junction and the pylorus. The incidence and mortality of gastric cancer have been declining in most developed countries. The age-adjusted risk fell 5% from 1985-1990.Clinical. Findings: Abdominal pain in an region, weakness and loss of appetite from 50 days, pain in lower limb from one month, nausea and vomiting, history of passage of black color stool (for two days, 50 days back), loss of weight five kg in last one month. Diagnostic Evaluation: Hb - 11.2 gm/dl Decreased , RBC 4.17cumm,CBC MCH- 22.6 Pico gm (decrease), Platelet -1.2 lakhs / cumm (decrease), Eosinophil- 9 % (increase )Monocytes-2% (decrease), KFT- sodium – 132 meq/L (decrease) LFT -bilirubin (conjugated) – 0.30 gm %(decrease)Bilirubin (unconjugated ) 0.33 gm %{ decrease}, CT scan, MRI, Upper GI endoscopy - showed abnormal mass ,Endoscopic ultrasound lesion as small as 2-3 mm in diameter, USG CECT Abdomen Report- showed enhancing wall thickening Involving body of gastric without obvious perigastric extension Or significant, consistent with gastric carcinoma. Histopathology report of gastrectomy specimen showed poorly Differentiated adenocarcinoma gastric mixed type – pT4a N1 M0. Therapeutic Interventions: Inf. Metrogyl 500 mg TDS, Inj. Amikacin 500 mg OD, Inj. Pantop 40 mg BD , Inj. Piptaz in 100ml NS 4.5 mg TDS, Inj. Levofloxacin 500 mg OD, Tab. Telma 40 mg OD, Inj. PCM in 100ml NS 500 mg TDS, Chemotherapy and Radiation therapy was also Done. Outcome: After treatment, the patient show improvement. His abdominal pain , nausea and vomiting , pain in lower limb were relieved and After all pharmacological, surgical and medical intervention , patient is now in stables condition his mental and it physical condition is improving and laboratory value are in normal range, and he is able to do his daily activities. Conclusion: My patient was admit in surgery Ward No- 28 , AVBRH with a known case of Gastric carcinoma and he had complaint of abdominal pain, nausea and vomiting ,pain in lower limb , weakness, black color stool and loss of appetite. After getting appropriate treatment his condition was improve.
