Cystic mesothelioma of the peritoneum is a wellrecognized, relatively benign, rare tumor of mesothelial origin.lP6 It is composed of multiple mesothelial-lined cysts embedded in a delicate fibrovascular ~t r o m a .~ Review of literature reveals that the most common location is in the pelvic peritoneum, particularly the cul-de-sac, uterus, and r e~t u m .~-~ We report a large, retroperitoneal cystic mesothelioma of the pelvis, which was nonadherent to contiguous viscera, ie., rectum and urinary bladder. The case from our institution had unusual characteristics, which were the patient's sex (male), age (62 years), location (retroperitoneal), and nature (free floating).
CASE REPORTA 62-year-old black man presented with a pelvic mass and irritative voiding symptoms. He had presented with similar problems two years ago. At that point, his workup included ultrasound and computed tomography (CT) scans of the abdomen and pelvis, which demonstrated a large, retroperitoneal multilocular complex, predominantly cystic mass in his pelvis, measuring 13 cm X 9 cm x 13 cm. The patient refused surgery at that time and was followed as an outpatient in the urology clinic. His symptoms worsened over a period of two years.Repeat ultrasound (Figure 1) and CT ( Figure 2) scans of his abdomen and pelvis 2 years after the initial presentation again demonstrated the same mass, which showed no interval change in dimensions or texture since the prior studies.
