Cardiovascular diseases (CVDs) are very common in modern society, such as atherosclerosis, hypertension, etc., which have a great impact on heart function. Therefore, hospitalization monitoring alone is far from enough. Long-term monitoring of the heart is needed in daily life. The technology of non-invasive monitoring of the cardiovascular system can meet the needs of long-term monitoring of the heart condition, helping to promote the improvement of lifestyle and daily care, reducing the overall risk of developing CVDs. The purpose of this study is to investigate the cardiac response after different exercises using a 3×3 demodulation scheme-based ballistocardiography (BCG) monitoring system. A fiber optic sensor (FOS)-based smart cushion is used to replace the traditional inconvenient electrocardiogram (ECG) for heart rate variability (HRV). The correlation between BCG inter-beat interval (IBI) and ECG IBI is 0.9862, and the RMSE is 0.0139. The BCG signal can assess cardiac contractility by analyzing RJ interval with ECG, which is a practical alternative to the pre-ejection period (PEP).
An optical fiber interferometer-based ballistocardiography (BCG) monitoring system aided with the IJK complex detection algorithm is proposed in this paper. A new phase modulation method based on a moving-coil transducer is developed to address the problem of signal fading in the optical fiber interferometer and keep the system in quadrature by the closed loop controller. As a result, a stable BCG signal without baseline drift can be obtained. This BCG monitor based on optical fiber interferometer using phase modulation method owns the advantages of compact, low-cost, portable, and user-friendly. In addition, an end-to-end modified U-net is developed to conduct pixel-wise classification in the BCG signal. This network can achieve high accuracy and shows its capability to segment IJK complex and body movement in the BCG signal. In conclusion, the proposed BCG monitoring system with IJK complex segmentation algorithm is potential and promising in healthcare applications.
Ballistocardiography (BCG) is a vibration signal related to cardiac activity, which can be obtained in a non-invasive way by optical fiber sensors. In this paper, we propose a modified generative adversarial network (GAN) to reconstruct BCG signals by solving signal fading problems in a Mach-Zehnder interferometer (MZI). Based on this algorithm, additional modulators and demodulators are not needed in the MZI, which reduces the cost and hardware complexity. The correlation between reconstructed BCG and reference BCG is 0.952 in test data. To further test the model performance, we collect special BCG signals including sinus arrhythmia data and post-exercise cardiac activities data, and analyze the reconstructed results. In conclusion, a BCG reconstruction algorithm is presented to solve the signal fading problem in the optical fiber interferometer innovatively, which greatly simplifies the BCG monitoring system.
IntroductionOrgan transplant recipients are at increased risk of developing pulmonary cryptococcosis (PC) due to weakened cell-mediated immunity caused by immunosuppressors. However, the nonspecific symptoms associated with PC can often lead to misdiagnosis and inappropriate treatment.MethodsWe conducted a retrospective analysis of data from 23 kidney transplant recipients with PC between April 2006 to January 2021.ResultsThe median time from transplantation to the diagnosis of pathology-proven PC 4.09 years. Seventeen patients presented respiratory symptoms, including sputum-producing cough and dyspnea. Additionally, three patients also developed central nervous system (CNS) infections. Chest CT scans frequently revealed nodule-shaped lesions, which can mimic lung carcinoma. Serological tests did not demonstrate any specific changes. Nine patients received surgical resection as treatment. Fourteen patients were treated with antifungal medication only. No recurrence was observed in all 23 patients.ConclusionOur study suggests that fever and sputum-producing cough are common symptoms of PC, and cryptococcal meningitis should not be excluded if corresponding symptoms occur. Fluconazole is a common and effective antifungal agent. Surgical resection should be considered for patients who do not respond well to antifungal therapy. Clinicians should be aware of these findings when evaluating transplant recipients with respiratory symptoms.
