Periodic monitoring and follow-up for the recurrence of hip instability is important in patients with cerebral palsy and a GMFCS level of IV or V.
Fracture is a neuromuscular damage due to tissue trauma or discontinue of bones. Fractures of the lower limbs due to accidents have the highest prevalence among other fractures in Indonesia. The aim of this study is to determine the characteristic of patients with femoral fracture in Department of Orthopedic RSUD Dr. Soetomo Surabaya.This research is retrospective observational. We analyzed medical records of all stay in patient in Department of Traumatic and Orthopedic RSUD Dr. Soetomo Surabaya during 1 January 2013 – 31 December 2016. Of the 972 enumerated, 112 subjects with femoral fracture were analyzed for study. The observed variable of patients’ profile include: sex, age, cause of fracture, type of wound, location of fracture, place of accident, time of accident.Our observational study during three years has concluded several findings as following. The most common cases of fracture is occurred at the age 15 – 24 with 40 cases (36%). Most of the cases is dominated by male gender (72%). Traffic accident (92%) is the major cause of fracture, which mostly occurred on the street (92%) at 06:01 A.M. until 12:00 A.M (28%). Closed wound (71%) is commonly found in femoral fracture. The most common type of femoral fracture has a closed wound (71%) and located in column of femur (46%). The most common cases of femoral fracture are occurred on male at the productive age due to traffic accident injury.
Fractures in children are important problem nowadays. Governmental census lists people under 17 years old as the largest contributor to fracture cases in Indonesia. The rapid increase in motor vehicle use, a hallmark of economic growth in developing countries, led to sharp increase in road saturation and consequently, rise in traffic related injuries. Variables involved in pediatric fractures needs to be identified and mapped to provide basis for the creation of better preventive measures to reduce problem before it happens. Thus, the purpose of this research is to create a profile of fracture in patients under the age of 17 years’ old at RSUD Dr Soetomo. The research is descriptive study which is performed by analyzing medical records in RSUD Dr Soetomo against the following: Age, Sex, Type of fracture, Cause of fracture, Time of fracture, Location of fracture, and Duration between admission and treatment. Data is then tabulated and converted into a bar graph for easier analysis. The research found that fractures in children most commonly occur at the 10-14 years’ age group (41.8%), happens more frequently in boys than in girls (69.5%), is dominated by closed fractures over open fractures (75.9%), and is most often caused by traffic accidents (60.9%). Consequently, the location in which fractures are most prevalent is the “street” (63.1%). It is also found that the highest incidence of fractures cases happens within the 12.01-18.00 time-span (19.1%) and is treated within the first 8 hours of admission to the hospital (79.4%).
Background: Extrapulmonary Tuberculosis is approximately 10% of all tuberculosis (TB) cases, and the musculoskeletal system’s involvement was only 2-4. The incidence of ankle TB was found to be less than 2% of all musculoskeletal TB. The rare occurrence, uncommon site, non-specific symptoms, lack of awareness frequently causes diagnosis difficulties.Case Report: A 2,5-year-old child came to the hospital with the main complaints of swelling and pain on the left ankle since one year ago. She was diagnosed with the ankle’s synovitis from the previous hospital and was given analgesics and a warm compress on the left ankle. However, the swelling of her left ankle increased during the last seven months, causing limitation of the left ankle movement, then she was admitted to the hospital for further examination. Radiologic examination revealed peripheral osseous erosions, reduced bone mineral content, and soft tissue swelling. A Mantoux test was positive with 15 mm induration and 25 mm erythema. Anti-tuberculous drugs (ATD) therapy was given with isoniazid 50 mg, rifampicin 75 mg, pyrazinamide 150 mg, and ethambutol 230 mg for two months, as an intensive phase therapy following rifampicin 75 mg and isoniazid 50 mg for ten months.Conclusion: Tuberculous arthritis of the ankle poses a diagnostic-difficulties, it should be considered especially in the endemic area. It is often misdiagnosed with other musculoskeletal conditions, which can delay the initiation of appropriate treatment. TB must be confirmed by histological pathology. A combination of ATD and surgery can lead to clinical improvement.
Background: Tuberculosis is one of serious health problems throughout the world, including in Indonesia. The incidence of tuberculosis continue to increase annually, especially the case of bone tuberculosis which is part of tuberculosis extra pulmonary as much as 10-20%.Case: A case report of 2 years old girl, with a neglected posterior hip dextra et causa coxitis tuberculosa dextra in Dr. Soetomo General Hospital, in period October 2013 until September 2015. Data were taken from the retrospective medical record through the anamnesis, physical findings, radiological, and laboratory examinations.Discussion: A 2 years old girl diagnosed as Neglected Posterior Hip dextra et causa Coxitis TB dextra, carried out of open biopsy, debridement, and open repositioning through the anterior approach Smith Peterson. Do installation of K-Wires and external fixation with hemispica. Given anti-tuberculosis drug for 12 months. In the monitoring for 2 years post-operatively, the patient reported an improvement.Conclusion: Hip Tuberculosis have a variety of mànifestasi similar to other diseases. Biopsy of the bone tissue lesions is a gold standard diagnosis for coxitis TB. However interpretation of the early picture of pelvic radiology can be a predictor outcome in the case of coxitis TB.
