Age, but not disease severity should be a factor when considering the transition needs of young adults with heart disease. The TRAQ has important limitations in the adult CHD/HTx population and a cardiac-specific measure of transition readiness is needed.
STCF-VADs can successfully bridge most pediatric patients to recovery, long-term device, or transplant, with an acceptable complication profile. Although these devices are designed for short-term support, longer support is possible and may serve as an alternative approach to patients not suitable for the current long-term devices.
Most parents felt ready for discharge following multidisciplinary teaching. Greater emphasis is needed on teaching families about services available in the community. Further study is required to determine which parents need additional support and education to avoid unanticipated challenges post discharge.
