MHE is a common complication in patients with cirrhosis of liver and higher CTP scores independently predict the presence of MHE. Patients with CTP class B and C have a higher risk of suffering from MHE than CTP class A. Screening of patients in CTP class B and C is likely to increase the MHE detection rates while saving time, although select CTP class A patients may also need screening in view of public safety or poor quality of life.
The subhepatic position of the cecum and appendix is a result of embryological reasons. Subhepatic appendicitis can cause diagnostic dilemmas. During the dissection of an adult male cadaver aged approximately 70 years, the subhepatic position of the cecum and appendix was noted. The appendix made a "U"-shaped bend and its tip was located in the paracolic position. The cecum had appendices epiploicae, and the terminal part of the ileum was retroperitoneal and had ascended vertically to the cecum from the right iliac fossa. Functionally, the sessile part of the ileum might restrict its peristaltic movements. The abnormal position of the terminal ileum might be mistaken for an ascending colon during laparoscopic surgery. The subhepatic position of the cecum and appendix might cause confusion in the diagnosis of acute appendicitis because the tenderness in such cases is not located at the McBurney's point.

SUMMARY:The porta hepatis of the liver is a very important area surgically. The knowledge of the variations in the structures passing through the porta hepatis might be of great help in reducing the risks of surgery in this area. In the current study 59 livers from South Indian cadavers were observed for the number and arrangement of the structures passing through the porta hepatis. In all the livers the portal vein was posteriormost and the hepatic duct was the anteriormost structure. The hepatic artery and its branches were in between the duct and the vein. The mean weight of the liver was 1.025 kg. The mean length of porta hepatis was 4.825 cm and the mean breadth was 2.433 cm. The number varied from one to three for the duct and vein but for the artery it was between one and four. In 51% of livers, only one vein and in 80% of cases only one duct passed through the porta hepatis. In 56% of cases two arteries passed through the porta hepatis.
