Paecilomyces is a common saprophyte found in air and food and is also associated with many types of human infections. Pneumonia due to Paecilomyces is rare and the clinical manifestations are unspecific. So distinguishing Paecilomyces infection clinically and prompt microbiological identification will help in adminiatering empirical antifungal therapy. We describe a case of atypical pneumonia by Paecilomyces lilacinus in a patient with interstitial lung disease and cor pulmonale. The female patient had breathlessness for over two years and was admitted to hospital as it worsened. Her X-ray revealed right and left ventricular failure. Sputum was sent for fungal bacterial culture and KOH for fungal elements. The Culture on Sabourouds Dextrose Agar grew purecolonies of Paecilomyces.
Background: Leprosy is eliminated from India in December 2005. But still leprosy cases with deformities are reported in developing countries. Multiple deformities are the major economical, social, and psycological problems in our country. In this situation, this study is undertaken to retrospectively analyse the various multiple deformities in hands, feet, and eyes in leprosy cases registered in our departmental records. Material and Methods: This study was an observational study, conducted in the department of Dermatology, Venereology and Leprosy of Rajah Muthiah Medical college and hospital. Ethical clearance was sought from institutional ethical committee. A retrospective analytical departmental record based study was done in leprosy patients, All cases of leprosy registered during Jan 2014 to Dec 2018 were evaluated in this study. Results: Out of 342 cases 154 cases diagnosed to have grade 2 deformity. Among these 154 cases 86 had multiple deformities. In our study with multiple deformities trophic ulcer with claw hand deformities (65.1%) were most commonly observed.
Conclusion:Multiple deformities were found in more than 25% of cases, with more number of grade 2 deformities. This warrants early diagnosis, prompt referral and management, and also education of the affected patients regarding protection of eyes, anaesthetic foot, and insensitive hands, to prevent progression of grade 1 deformities to grade 2 deformities.
