Primary small cell carcinoma (SCC) of the pancreas is a rare disease with poor prognosis. Very few cases have been reported in the literature. It is a type of poorly differentiated variety of neuroendocrine tumours of the pancreas with specific immunohistochemical markers. Imaging is not diagnostic for disease, and diagnosis is mainly by biopsy. We report a rare case of SCC of the pancreas who presented with features of obstructive jaundice without any paraneoplastic features. The patient is planned for palliative chemotherapy because of metastasis and is under regular follow-up.
Spontaneous pneumomediastinum (SPM) refers to the presence of air in the mediastinum without any obvious cause. It is an uncommon condition occurring due to alveolar rupture as a result of increased intrathoracic pressure. It is commonly seen in young males, patients with known asthmatic disorders and pulmonary diseases. We report a rare case of SPM in a young healthy male without any significant past history. The patient was managed conservatively and discharged.
The gallbladder is a well-protected organ. Hence, injury to the gallbladder following blunt trauma is infrequent. The clinical presentation of gallbladder injury is variable, resulting in a delay in diagnosis and treatment. Isolated gallbladder perforation is a diagnosis of exclusion. Ultrasonography and contrast-enhanced CT are valuable assets in diagnosis. Cholecystectomy remains the procedure of choice. Mortality and morbidity is high in late presentations with biliary and bacterial peritonitis. It is thus necessary that the clinician be aware of this entity to have a high index of suspicion and to treat early.
Isolated penile Fournier’s gangrene is a rare condition causing significant morbidity to the patients. It occurs due to urinary tract infection or trauma. We describe an elderly male who presented with blackish discoloration of the skin of the penis with fever. Examination revealed a necrotic patch over the shaft of the penis without any other foci of infection around the perineum. Penis was debrided, and the antibiogram showed Escherichia coli sensitive to amikacin. He received seven days of antibiotics, and the wound granulated well. A split skin graft was used to cover the wound to prevent contraction. This unusual presentation requires expeditious management to prevent mortality and morbidity.
