The origin of the recipient is one of the recipient-related factors that statistically significantly affects the outcomes of kidney transplantation, but this factor is not so relevant in our country. The article presents the first experience of kidney transplantation in a patient of African descent at the N.I. N.V. Sklifosovsky.Material and methods. Allogeneic renal graft transplantation from a post-mortem donor was performed in a 60-year-old African male patient with stage 5 chronic kidney disease as a result of hypertensive nephroangiosclerosis.Results. The early postoperative period was complicated by the development of acute cellular graft rejection grade 1a-b according to Banff. Anti-crisis therapy was performed with some positive effect, however, in the late postoperative period, a loss of renal graft function was noted, and the patient was returned to renal replacement therapy with program hemodialysis.Conclusion. Patients of African ancestry are at increased risk of adverse outcomes due to immunological causes and require more careful immunological selection of a donor organ, an enhanced immunosuppressive regimen, and close monitoring after transplantation.
The main theoretical approaches to the formation of a system of the indicators that determine the generation of economic value added, as the main indicator that reflects the efficiency of the economic entity and its reorganization, are considered. To determine the factors that have the greatest impact on the generation of economic value added, a provisional list of the most used indicators (both dynamic and static) applied to assess and analyze the performance of activity of an economic entity is comprised. Among the indicators are identified those that are correlation-independent (key), based on the generation of a reliable sample when calculating the efficiency and performance of economic entities in the industrial sector. On this grounds, a simulation modeling was performed, which results were indicated feedback paths, covering the key characteristics of the activity of economic entity. It is determined that the factors, influencing the generation of value added the most, are related to ensuring financial stability, efficiency of return on investment, the structure of fixed assets, the structure of financing the activities of the economic entity. The carried out analysis of the identified relations helped to allocate the key factors, which have the greatest impact on the efficiency of activities, and, accordingly, determine the directions of reorganization of the economic entity. Making use of this approach allows to determine at the empirical level the most appropriate combinations of management technologies with the financial and economic instrumentarium for carrying out a reorganization of the economic entity and forecasting the consequences of their application. The necessity of using the indicator of economic value added as a target criterion for assessing the operation and development of an economic entity is proved.
Introduction. Infectious complications contribute to a significant decrease in graft and recipient survival rates. The article describes a case report of transplantation of the primary infected kidney transplant. Material and methods. A 33-year-old patient with type 1 diabetes mellitus and end-stage renal disease underwent kidney transplantation. The deceased donor was a 46-year-old man with a confirmed brain death as a result of acute cerebrovascular accident. Results. The early postoperative period was complicated by the development of primary infection of kidney transplant. Despite the ongoing treatment aimed at preserving the transplant, we had to remove it in order to prevent the development of further complications. Conclusion. Kidney transplantation improves the quality of life of patients with end-stage renal disease. In case of uncontrolled course of the infectious process after primary infected graft transplantation, it is necessary to perform transplantectomy in a timely manner in order to save the recipient’s life, since the graft itself is the source of infection.
The main theoretic approaches to defining the concept of «reshaping a business entity» are considered. The study of modern tools and approaches to reforming socio-economic systems (reengineering, restructuring, modernization, optimization, etc.) has shown their limitation and concentration on certain components of their activities or management systems. A theoretical and morphological study of related definitions that characterize the transformation and reform of economic entities is carried out, their interrelation and interdependence are determined. The theoretical and methodological basis and stages of reshaping a business entity are substantiated, taking into account the peculiarities of its technological network structure, consumer value chains, management system, financial flows and key competencies within its activity. The key components of organizational and economic support for reshaping the activity of a business entity in terms of profile determination are identified, namely: management, main and auxillary activities, diagnosis and evaluation, the formation of the means and tools used for reshaping. Given the key parameter of business entities, i.e. the return on capital brought in by investors, it is proved necessary to use the indicator of economic value added as a target criterion for assessing the functioning and development of such entities and, consequently, necessary to reshape its activity. In addition, this approach allows predicting the consequences of introducing tools of reshaping, thus making it possible to assess its feasibility and set up the most effective organizational and economic measures.
