Objective of the study. To investigate the effect of triazino-indole derivative on the parameters of glycolysis in the endolymph during preventive and therapeutic use on the model of acoustic trauma. Experimental animals and methods. The experiments were carried out on 82 mice, female F1 hybrids of the CBA and C57BL/6 lines weighing at least 17 g at the age of 4–12 weeks. When simulating an acute acoustic trauma, an animal was exposed to white noise with a level of 107 dB SPL and frequency band 3–100,000 kHz. For therapeutic purposes, a single intravenous injection of a triazino-indole derivative was carried out immediately after noise exposure. The drug was administered at doses of 5, 7, and 10 mg/kg of animal weight. For prophylactic purposes, the intramuscular drug injection was administered before the acoustic trauma application. The drug was administered at doses of 5, 7, and 10 mg/kg of animal weight once a day for 3 days before noise exposure. The Cytoflavin was a reference drug. A normal saline was administered to the control group. When using the acoustic trauma models, the metabolic processes level in the cochlea was assessed by the glucose and lactic acid (lactate) content in the labyrinth fluid before and 2 hours after pathological exposure. Results and discussion. The triazino-indole derivative, both during preventive and therapeutic administration, significantly corrects changes in the glycolysis (glucose, lactate) indicators in the endolymph, thereby improving the bioenergetics of the inner ear cells. The best effect is observed when the drug is administered at a dose of 10 mg/kg. When compared with Cytoflavin, the antihypoxic effect of the triazino-indole derivative is more pronounced.
For a long time for otolaryngologists, the issues of identifying and treating such a formidable disease as chronic suppurative otitis media (CSOM) have been topical. This disease still haunts many specialists, as it leads to acquired hearing loss in people of working age. Only in the last ten years, the number of children of younger age groups with already manifested acquired hearing loss has increased. Along with the correct and timely diagnosis, it is very important to cure this disease once and for all, since exacerbations often disrupt not only the performance, but also the quality of life of the patient. The gold standard for the treatment of patients with CHSO is surgical – tympanoplasty. In the process of studying the pathological process in chronic suppurative otitis media and methods of its more effective surgical treatment, the results of tympanoplasty were analyzed. An illustrative clinical example has been chosen, with the help of which it is possible to trace all the stages of surgical intervention in chronic suppurative otitis media. An otoscopic examination was carried out at several stages of observation of this patient, as well as the determination of auditory function. The presented clinical case shows step by step the effectiveness of the scheme of surgical treatment and postoperative management. Thus, these steps are applicable to any myringoplasty with tympanic membrane defects of any size. It is tympanoplasty and control in the postoperative period that makes it possible to completely close the defect of the tympanic membrane and prevent chronic otitis media from recurring.
In recent decades, among diseases of the nose and paranasal sinuses, an increase in the number of people with perforations of the nasal septum has been noticeable. To date, the most effective method of treating perforations is operational. Currently, surgeons’ work on closing defects of the nasal septum does not always end with a positive result due to the fact that the transplant materials used in plastic surgery of perforations are often rejected some time after surgery and the perforations reappear. In our study, we treated 80 patients with septum defects in the age range from 18 to 63, for whom septum perforation was closed using transplant materials: fascia treated with Alloplant technology and allogeneic cartilage treated with Alloplant technology and without application. The septum defect closure performance was determined after 1 month and 1 year after surgery. The study revealed that plastic surgery of nasal septum perforations using transplant materials processed using Alloplant technology using the anatomical, clinical, and functional results of treatment one month and one year after surgery has a more effective method than the classical treatment method and leads to fast and full return of all functions of the mucous membrane of the nasal cavity.
Relevance in research infectious and inflammatory diseases of the upper respiratory tract caused by high prevalence, variability of clinical manifestations and severity of the disease with the risk of developing complications. Respiratory diseases are a cause of functional and morphological disorders of the respiratory tract mucosa which is a protective barrier against various damaging agents. It is necessary to use complex drugs that preserve and restore the physiological activity of the mucous membrane as much as possible. In this regard, there is a great interest of plant medical product (extract of the roots of pelargonium sidovid is the chief active ingredient) which causes stimulation of nonspecific protective mechanisms, the beating frequency of the cilia of the ciliated epithelium, the activity of NK cells, phagocytes, modulation of the synthesis of interferon and proinflammatory cytokines, the expression of adhesive molecules, chemotaxis. We can notice moderate direct antibacterial and antiviral properties of extract. The pharmacotherapeutic efficacy of the plant medical product containing an extract of pelargonium sidovid roots was analysed in our research to treatment of respiratory diseases. There is a literature review about efficacy and safety preparations containing liquid extract of pelargonium sidovid roots. The extract effectiveness is confirmed by the results of clinical investigation: children and adults are digest the drug, greatly facilitates the course of diseases, prevents the development of complications, and reduces the duration of acute respiratory infections. Extract of the roots of pelargonium sidovid is easy to use and dosage. All these properties make it possible to include this medical product in the complex therapy of inflammatory diseases of the respiratory tract.
