Aim. The paper aims at analyzing the state of cancer epidemiology in Ukraine for the past decade specifying the age, gender, territorial and nosological peculiarities. Materials and Methods. The study was conducted on the basis of the data analysis of the National cancer register of Ukraine for the 10-year period: starting from 2010 till 2019 years. The research was directed at studying and analyzing morbidity and mortality levels caused by malignant neoplasms, as well as age, gender, territorial and nosological peculiarities and the prevalence of nosological forms. Applied statistical methods include the analysis of the average and relative values, the calculation of indices of time series: growth rate and absolute growth, structural-and-logical analysis and systemic approach. Results and Discussion. The study has found that during the analyzed period of 2010-2019 years, the morbidity from malignant neoplasm in Ukraine increased, while the mortality level caused by oncologic pathology decreased. The morbidity and mortality among male population was higher in comparison with the female individuals. Over 50% of patients with firstly diagnosed "Malignant neoplasm" were identified in the early stages, but the share of detected patients with neglected form of oncologic pathology was increasing every year. One-year survival among the general population of patients with malignant neoplasms accounted 66,4-72,9%. The highest rates of morbidity have been fixed among male and female population, aged 70-79 years, while the highest death rate - among older individuals, 75-84 years of age. It has been defined that the age of 30-64 years wass characterized by high levels of morbidity and mortality rates in every sequential five-year group in comparison with the previous one. The hematological oncologic diseases were dominant in the structure of morbidity and mortality in young children and teenagers under 17 years. For the age category of 30-74 years, in female population the most common was breast cancer, in males - the cancer of the trachea, bronchi, lungs. For the analyzed period, Kirovograd Region occupies a leading position by morbidity level, while the Zaporizhzhia, Kirovograd and Dnipro Regions - are leaders in the mortality rate from malignant neoplasms. Conclusions. The analytical study of the oncologic and epidemiologic situation is an integral part in the development of an optimized system of oncologic care for Ukrainian population as it allowed to determine the age-gender and territorial groups, which require priory administrative attention concerning diagnostic and therapeutic decisions in oncology.
The quality of life of patients is one of the indicators of the medical care effectiveness for cancer patients. In order to study the main aspects of the quality of life of patients with breast cancer, a survey of 400 patients at the stage of their inpatient treatment in oncological institutions in nine regions of Ukraine (Chernihiv, Zaporizhia, Dnipropetrovsk, Kyiv, Poltava, Khmelnytsky, Ivano-Frankivsk, Zakarpattia and Lviv) using the EORTC QLQ-C30 and EORTC QLQ-BR45 questionnaires was carried out. In Ukraine, as of the pre-war period (November 2021 – February 2022), the quality of life of breast cancer patients equaled 53.22 on a 100-point scale. The most pronounced impact of breast cancer on the quality of life of the patient was observed in the psycho-emotional sphere (71.26), role functioning (70.48), views on future prospects (30.95). Patients complained of financial difficulties (44.19), fatigue (35.16), sleep disturbances (33.00), hair loss (43.41), as well as decreased sexual function (16.06) and pleasure (18.16 points). Given the low quality of life of breast cancer patients, they need psychological support, which should be aimed at adapting to the manifestations of the disease and future prospects, and the quality of life of patients should be one of the indicators in the course of treatment and providing psychological support. Since patients who have received social and psychological support from their doctors and friends/family have higher quality of life, we consider it necessary to focus on developing a comprehensive program to support breast cancer patients in collaboration with society, family and workplace.
Annotation. The fight against oncological diseases remains one of the priority directions of the health care system of Ukraine, yet this process has become much more complicated in the conditions of martial law. In order to study the state of providing oncological care to the population of Ukraine in wartime conditions, information from the National Health Service of Ukraine was studied. The distribution of oncological treatment service providers by regions and by ownership was performed, the level of hospitalizations per 100,000 individuals and the available bed fund per 1 million individuals were analyzed. Statistical methods (analysis of relative values, calculation of the correlation coefficient), structural-logical analysis and systematic approach are applied. The research established that, as of July 2022, 103 institutions (including 7 private ones) provide chemotherapeutic treatment services, and 35 institutions (including 2 private ones) provide radiological treatment services. The largest number of chemotherapeutic treatment providers per 1 million individuals is located in Kirovohrad region, the smallest – in Kyiv region. Meanwhile, Zaporizhia and Kirovohrad regions are characterized by the largest number of radiological treatment providers per 1 million individuals, while Odesa and Kharkiv regions have the lowest number. The largest number of oncology beds per 1 million individuals and the largest number of hospitalizations per 100,000 individuals were registered in the city of Kyiv and in Poltava region, the lowest – in Luhansk region. When organizing the provision of oncological care to the population in wartime conditions, the factor of cross-border treatment, as well as the use of telemedicine programs, must be taken into account. Further research is required to study the impact of the military conflict on oncology risk, the level of morbidity, mortality and neglect of cancer in the population of Ukraine.
Annotation. Oncology remains a global problem for humanity. International efforts to overcome it aim at implementing programs for prevention and early detection of neoplasms. The article is devoted to the study and analysis of the results of the implementation of programs to fight oncological diseases in Ukraine in the period until 2022. We have performed a one-time exploratory and descriptive research by the means of processing the materials of state and targeted regional programs to fight oncological diseases, which were implemented in the regions of the center of the Ukraine (Dnipropetrovsk, Kirovohrad, Poltava, Vinnytsia, Cherkasy regions), and we have assessed the level of their funding and implementation results. The study was conducted on the basis of information provided by the Health Departments of regional state administrations in 2021, as well as data obtained from the websites of these Health Departments. Statistical methods (analysis of relative values, calculation of absolute growth rate), structural-logical analysis and systematic approach were used. The research established that the implementation of programs for the prevention and control of oncological diseases made it possible to achieve positive dynamics of the main indicators of providing oncological care to the population (decrease in the one-year mortality rate, decrease in the neglect rate, increase in the percentage of patients covered by special treatment). However, against the background of positive trends, the level of funding programs in the field of cancer prevention remains insufficient. According to the findings of our research, certain gaps in oncology programs were revealed, namely: insufficient attention to the development of palliative medical care for cancer patients and problems of oncology in children. The obtained data serve as the basis for the development of measures to improve the work of the oncology service in Ukraine.
Introduction. Prostate cancer (PC) is one of the most common cancers among men over 50. Nowadays in Ukraine, the number of new cases of PC tends to increase according to statistics. Objectives. The aim of the study is to identify the main problems in the life quality of patients with PC (C61) to optimize medical care for them. Methods. The main aspects of the quality of life of PC patients are studied by means of EORTC QLQ-C30 and EORTC QLQ-PR25 questionnaires. Results. In Ukraine, as of the pre-war period (November 2021 – February 2022), the quality of life of patients with PC amounted to 52.31 on a 100-point scale. The worst indicators within the functional scale QLQ-C30 belonged to the subscale “Role functioning” (65.44). According to the QLQ-C30 symptoms scale, the highest score among the complaints of Ukrainian patients is fatigue (48.58). Given the results of our research on the QLQ-PR25 symptom scale, patients with PC were the most worried about “Urinary symptoms”, with a score of 38.54 out of 100. According to the functional scale QLQ-PR25, the worst indicators were for “Sexual activity”, which is equal to 32.22 points on a 100-point scale. Conclusions. It is important to create socio-psychological support for patients with PC and their families, which will include the development and implementation of standardized psychological care at all stages of diagnosis, treatment and rehabilitation.
