This year we have piloted the Sustainability in Quality Improvement framework (SusQI) within our quality improvement teaching. We would now like to capture your personal experiences and honest views through a survey in order to evaluate and improve it. This survey is completely anonymous and participation is voluntary. You will be not harmed in any way by your decision about whether or not to complete the survey.
