BACKGROUND The relationship of the various positions of appendix to its clinical picture and course in the patient is a subject of controversy. The objective is to study the correlation between anatomical position of appendix and its varied clinical presentation and also to determine the frequency of different positions of appendix in operated cases of acute appendicitis.

A 12-year-old girl with abdominal pain and fever underwent urgent surgery. As was suspected 15 on computed tomographic scan, a large diverticular mass adherent to the cecum was identified. A 16 diverticulectomy was performed. We report this rare case of a giant cecal diverticulum and provide an 17 overview of known literature. Acute abdominal pain is a very common reason for 21 consulting a pediatric surgical emergency care unit. 22However, the diagnostics and treatment can be challenging. Because of the clinical presentation and inconclusive 56 imaging, the patient was taken to the operating theater. 57Laparoscopy was converted to an infraumbilical median 58 laparotomy because of visualization of ischemic bowel. We 59 discovered a giant diverticulum originating from the 60 antimesenteric cecal wall (Fig. 3). This mobile thin-walled 61 structure of 116 g measured 30 cm in length and 7 cm in 62 diameter at its broadest point.
