Cause of uncertain knee-complaints in children can be foreign bodies which penetrated unnoticed the joint cavity. Here we report an unusual injury of this kind. A glass-foreign body intruded the knee joint in childhood. Complaints occurred for the first time in adult age. The diagnostic and therapeutic measures in such injuries are shown. At removal of the foreign body tetanus immunisation is necessary.
In the period from 1977 to 1982 78 children were treated for supracondylar fractures of the humerus at the Berufsgenossenschaftliche Unfallklinik Frankfurt/Main. Indications and methods for operative and conservative treatment are described. The results of therapy were reviewed by means of a questionnaire or reexamination in 68 patients with regard to function, elbow deformity and nerve lesions. Fractures of a grade I severity are to be treated conservatively by immobilisation in a POP-cast. Only in exceptional cases of grade II severity can a closed reduction be recommended. All remaining types of fractures of grade II and III severity should primarily be treated by open operations within 8 hours of the accident. Under these conditions the risk of infection is no higher than with percutaneous Kirschner-wire-osteosynthesis. Complications such as appreciable varus deformity, poor function and nerve lesions can largely be avoided in this way. The so-called extension-treatment offers no advantages in comparison with operative treatment.
Skeletal muscles is an uncommon site for metastatic spreading of malignancies such as carcinoma of the lung. Thus, to date, there have been only a small number of reports of carcinoma of the lung spreading to the skeletal muscles. This case study reports the rare combination of a rotator cuff lesion of the shoulder with the late occurrence of the metastasis of a squamous cell carcinoma of the lung in the ipsilateral deltoid muscle. The problems that result from this combination concerning diagnosis and therapy are discussed.
The report concerns a patient with extended osteolytic metastasis of the pelvic region including the left hip joint with a primary kidney carcinoma. A simple fixateur-exterior apparatus for arthrodesis of the hip joint including immediate painlessness is presented. The procedure is relatively simple and with little strain for the patient. Due to the overall poor prognosis of the underlying disease, in our opinion the most simple procedure should be chosen over any complex external constructions or invasive procedures.
We report a rare case of complete ureteric avulsion as the result of indirect force and discuss the mechanism of injury in the light of relevant literature. The special problems of this injury with regard to diagnosis and therapy are explored. Every polytraumatized patient with involvement of the trunk should have an IVA at the earliest possible moment as a basic investigation.
