Resumen Introducción y objetivos:Revisar las posibilidades de diagnóstico en un paciente que tiene taquicardia de QRS estrecho con R-R variable Métodos y material: Se presenta el caso de una paciente de 38 años que acudió por crisis frecuentes de palpitaciones que inducían mareo y sudoración y, en dos ocasiones, síncope. Se indicaron tratamientos sucesivos con carvedilol y verapamil sin éxito y fue sometida a ablación. Resultados: En el estudio electrofisiológico, con un extraestímulo, se indujo una taquicardia irregular, de QRS estrecho que se discute. Conclusiones: La coexistencia de varios mecanismos de taquicardia puede originar taquicardia de QRS estrecho y R-R irregular.Palabras clave: Taquicardia supraventricular. Ablación por catéter. Abstract Background and objective: We here review the diagnostic possibilities for one patient with narrow complex irregular tachycardia. Patients and methods: We report the case of a 38-year-old female patient with frequent paroxysms of tachycardia, dizziness diaphoresis and syncope.The patient received carvedilol and verapamil without success and was referred to for ablation. Results: During the electrophysiological study, programmed atrial stimulation induced an irregular narrow-complex tachycardia which is the subject of discussion of this report. Conclusions: Several tachycardia circuits can coexist and can induce narrow complex irregular tachycardia.
