Prostate leiomyosarcoma is an extremely rare and very aggressive neoplasm that represents less than 0.1% of primary malignant tumors of the prostate. We present a patient with primary leiomyosarcoma of prostate and examine the cases reported in the literature to discuss the clinical, diagnostic and therapeutic aspects of this rare tumor. Median survival was estimated at 17 months (95% CI 20.7–43.7 months) and the actuarial survival rates at 1, 3 and 5 years were 68%, 34% and 26%, respectively. The only predictors of long-term survival were negative surgical margins and the absence of metastatic disease at presentation. A multidisciplinary approach is necessary for the proper management of this terrible entity. Surgery with or without chemotherapy seems to be the main therapeutic method for operable leimyosarcomas, but in general there is no consensus on the best therapeutic approach. Most cases are diagnosed at an advanced stage of the disease. A global multicenter trial is needed to find therapies that would improve the prognosis.
Since December 2019, the COVID-19 pandemic has known a rapid and distressing spread. Little is known about the sexual impact on infected patients. However, several studies have reported a significant psychological impact on the population.The objective of our study is to assess the impact of the COVID19 pandemic on the sexuality of individuals who have been infected with the virus.Our study focuses on a population of 50 Moroccan patients initially declared to have COVID-19, using a multiple choice questionnaire on sexuality before and after infection with the virus. All participants were part of a couple in which at least one of the two partners had been infected with SARS-CoV-2.The results showed that all subjects were sexually active before their infection, of which 20% were dissatisfied with their sex life. At the time of our study, 78% of the recovered patients had not resumed sexual activity. 60% said they were anxious about having sex and 81% said the pandemic had altered their sex life. Only one patient reports an increase in libido and an improvement in the quality of orgasm. A couple reported having resumed sexual activity but with a change in their relationships.Anxiety and fear of infecting the other are major concerns, which affect the resumption of normal sexual activity and which sometimes lead to a change in sexual practices.
Le Phéochromocytome extra-surrénalien ou paragangliome est une tumeur rare. Il dérive de la crête neurale du système neuroendocrinien et il est souvent sécrétant. La plupart de ces tumeurs sont situées dans les ganglions sympathiques abdominaux, y compris l'organe de Zuckerkandl adjacent à la bifurcation de l'aorte abdominale. Les Formes non fonctionnelles sont rares: moins de 50 cas sont rapportés dans la littérature. Nous rapportons le cas d'un paragangliome non sécrétant de l'organe de Zuckerkandl opéré au sein de notre service d'urologie ainsi qu'une revue de la littérature afin de discuter les différents aspects diagnostiques et thérapeutiques de cette entité tumorale rare.
There is a many example of diseases of blood vessels involving serious diagnostic difficulties and sometimes the etiologic factors may be only presumed. Varicose veins rarely affect the penis, and we were unable to find in dermatologic and urologic literature reports of a similar case. We report here a case of a 26-year-old man with a history of a penis getting bigger since his adolescent life associated with minor pain, discomfort, and a primitive erectile dysfunction. After a complete medical history, physical examination and ultrasonic doppler color investigation, the diagnosis of acquired phlebectasia of the penis was confirmed. The patient was treated surgically by ligaturing and section of the voluminous dorsal varicose veins of the penis and reducing the ventral scrotal skin, with successful aesthetic and functional results, with a follow up more than 3 months. Varicose veins of the penis are an exceptional pathology. This condition may not need any treatment. However, when varicose veins cause great discomfort, pain, aesthetic or sexual issues, surgery is the best option of the patient.
Intravesical migration of intra uterine device is extremely rare. Few cases only have been reported in the litterature. This migration may cause many complications including vesical stone.Device migration may be secondary to a vesicovaginal fistula, or uterin perforation or a trans-uretral insertion of the IUD.The case reported is one of a 52year-old woman with lower urinary tract symptoms that occured 25 years after the insertion of the IUD, that has been complicated with a vesical stone. The constitution of the lithiasis, has been though, the main consequence of thisforeign body.
