At the actual stage of becoming of Russian stomatology, the problem of sustainable development of rural stomatological care, as system of local formations of complex medical social nature belongs to national priorities and is considered as one of most important directions of public social policy. The stomatological health of rural population is to be considered as indicator of national stomatological health. This is confirmed by spatial structure: rural territories (inhabited territories outside cities) cover 2/3 of territory of the Russian Federation with population of 37.3 million or 1/4 of total population. The spatial structure of the Belgorod Oblast is reliably similar to the all-Russian one. The number of national and foreign studies proved that accessibility, quality and timeliness of insurance (state) stomatology is lower for rural residents that can be considered as one of signs of social inequality. Depending on the social economic status of region, signs of social inequality in the stomatology field are conditioned by broad spectrum of factors. Some of them are discussed in the article.
The article suggests a hypothesis that the implementation of the Zemsky Doctor program does not lead to a sufficient increase in the attractiveness of young specialists moving to rural areas, which in turn reduces the availability of oral sanitation in the CRH and indicates the presence of a dental aspect of social inequality. To confirm or refute the truth of the hypothesis, two tasks are set: 1. To assess the role of the Medical Institute of the Belgorod State National Research University as a factor determining the settlement of dentists in rural areas. 2. To analyze the dynamics of the number of fillings placed for pulpitis and periodontitis in Prokhorovsky, Rakityansky, Volokonovsky dental departments for 2017 and 2021. The solution of the tasks set allowed us to speak about the truth of the formulated hypothesis. The revealed features of the personnel of the dental departments of the CRH indicate the absence of a significant influence of the medical institute on the staffing of the CRH by young specialists. The results obtained in the process of comparative evaluation of the number of fillings made for complicated caries revealed a widespread decrease in the intensity of endodontic treatment observed in different CRH.
Based on the literature data and own research performed at the Department of Orthopedic Dentistry of the Medical Institute of the National Research University of Belgorod State University, a review of the methods of information and communication interaction of dental institutions with patients of retirement and pre-retirement age in the context of an increase in average life expectancy and ongoing pension reform is carried out. Elements of strategies of a medical institution that enhance its attractiveness for people of older age groups are presented, as well as the corresponding elements of corporate culture and aspects that allow clarifying the opinions of patients of these age groups on the quality of dental services provided; formulated requirements for employees of medical institutions relevant in the current conditions.
