The aim: Improving treatment outcomes in patients with segment tibial bone tissue loss after fractures by improving the ring fixators technology. Materials and methods: The study includes the data of 77 patients. The patients were distributed between two groups: the main and control one, using random numbers table. The inclusion criteria were: patients’ consent, their age over 18 years, tissue inflammation episodes, fibular defects 4-5cm in size and disseminated scar lesions. RF was applied with the proposed improvements. Finally, a semi-rigid external fusion system Softcast / Scotchcast was used to prevent deformation of the regenerate and create optimal conditions for fusion of fragments. Results: The following results were obtained for the Modified Functional Evaluation System by Karlstrom-Olerud: positive treatment outcomes in the main group made up 97.1%, and 92.9% in the control one. Though, the specific gravity of good and excellent outcomes in the main group was 82.8%, while in the comparison group – just 45.2% Conclusions: Along with considerable positive advantages of the “Ilizarov’s” apparatus, some disadvantages have been noted. Elimination of the drawbacks is stipulated for the practical needs, which requires further scientific study.
The aim: To improve the psoriatic arthritis treatment outcomes by establishing and implementing into practice operative treatment. Materials and methods: The study basis is represented with the results of examination and treatment of 252 patients with psoriatic arthritis. The biopsy samples from the problem psoriatic eruption and intact skins regions, received from 74 patients, were studied. Results: The study has established presence of Staphylococcus aureus in affected skin regions. The authors present their classification of structural muscular-skeletal disorders associated with psoriatic arthritis, which will substantiate the volume and character of operative interventions. Conclusions: Detection of numerous opportunistic pathogenic microorganism colonies stipulates for the necessary skin debridement while planning further operative treatment, aimed at correction of anatomical and biological abnormalities. The operative intervention volume depends on the severity of structural dysfunctions.
Dislocations of the clavicle are one of the most common injuries in people of different age groups, which require prompt diagnosis and timely treatment to prevent the development of complications, therefore, acknowledgment of general practitioners-family doctors in diagnosis and medical care for patients with dislocations of the clavicle is a necessary component of the professional competence. Aim: to provide up-to-date information for distance self-learning of general practitioners – family doctors on the diagnosis and treatment of clavicle dislocations and conduct a test assessment of the acquired knowledge. Materials and methods. The analysis of scientific and methodological medical literature and clinical cases from the practice of orthopedic traumatologists, concerning anatomy, classification, mechanisms of development, diagnosis, clinical picture, conservative and principles of surgical treatment of patients with dislocated clavicle is given in the article. Results. A team of authors – orthopedic traumatologists – presented a review of the literature data and clinical cases of dislocation of the clavicle, on the basis of which educational material was formed for distance self-learning of the general practitioners – family doctors. The modern data on the mechanisms of development of dislocation of the clavicle, classification, clinical picture, depending on the type of dislocation are presented; recommendations for conservative treatment and indications for surgical intervention are given. Tests for self-control of knowledge after studying the presented material are given. Conclusions. Knowledge of the management of patients with clavicle dislocation is a necessary component of the formation of professional competencies of a general practitioner – family doctor. The presented article for distance learning provides all the necessary up-to-date information to provide such knowledge.
