Background/Aim: Segmental fractures represent complex tibial injuries, featuring a unique fracture type that is most commonly caused by a high-energy trauma. These fractures are considered to be a treatment challenge for orthopaedic surgeons due to their sporadic presentation, wide zone of soft tissue injury, and increased rate of complications. They are characterised by highly unstable intermediary segment and high rate of open fractures. The method of Ilizarov with its characteristics could offer many advantages over the existing operative techniques. This method, using a percutaneous approach, minimizes the intraoperative trauma and avoids the additional compromising of the biological environment at the fracture site. The aim of this study is to evaluate the results of Ilizarov fixator in treatment of segmental tibial fractures. Methods: We analysed 30 patients treated with an Ilizarov fixator between 2012 and 2017. Average age was 36 years (from 24 to 65). The most common mechanism of injury was a road traffic accident. Open fractures were noted in 22 cases. All fractures were reduced using indirect percutaneous techniques with a great focus on achieving the correct length, rotation and axial alignment of fragments. All patients were advised to bear weight as tolerated from the second postoperative day. Bone healing and functional results were evaluated according to criteria established by the Association for the Study and Application of the Method of Ilizarov. Results: Bone healing was achieved in all patients. The average time to union was 25 weeks (19 to 36 weeks). The bone results were excellent in 23 patients, good in five patients and fair in two patients. The functional results were excellent in 22 cases, good in 5 and fair in three cases. Eight patients had minor pin-tract infections, successfully treated with oral antibiotics. Patients were without any major complications. Conclusion: Ilizarov method is a safe and efficient treatment modality for segmental tibial fractures.
Osteons are the basic units of bone tissue. Four types of bone cells are responsible for building the bone matrix, maintaining it and remodeling the bone. Those are osteoprogenitor cells, osteoblasts, osteocytes, and osteoclasts. The main role of the bone is in the construction of the skeleton that provides support and enables the maintenance of a constant body shape and movement, which is achieved by a complex system of levers influenced by muscle forces at the attachment points. A fracture leads to the process of inflammation, healing and, remodeling that can restore the injured bone to its original state. Nowadays, we are more and more often faced with defects of different size and etiology, which increase the need for bone transplantation. This procedure involves a surgical technique that places a new bone or a suitable substitute between or around the fractured fragments or in the area of the defect to promote bone healing. Bone transplantation is possible because bone tissue, unlike most other tissues, has the ability to fully regenerate if it is provided with adequate conditions and the space in which it will grow. The basic biological mechanisms that justify and explain the principle and utilization of bone grafting are osteoconduction, osteoinduction, and osteogenesis, but the idea of osteopromotion is also worth mentioning. Autografts represent the "gold standard", but due to an increased need allografts from bone banks are primarily used nowadays, while the development of synthetic replacements is the primary research interest, especially the development of the form which could be applied with the use of minimally invasive techniques.
The aim of this study was to establish whether older patients with skin melanoma (in an analyzed group of 189 patients treated at the Institute for Oncology and Radiology of Serbia from 2004 to 2008), have worse survival compared to younger patients. In 100 men and 89 women with an average age of 58.9 years, the following parameters were observed and statistically analyzed in SPSS: gender, age, localization, tumor thickness, ulceration, lymphonodal status and invasion level. In the four age subgroups - quartiles, the best survival was shown in patients <50 years (85.7%), and the worst in patients >70 years (76.1%). Patients without ulcerations, with negative lymph nodes, thin melanomas and Clark levels I and II had significantly better survival outcomes. Although the results showed no statistical significance of age as a prognostic factor in the survival of patients with skin melanoma, further research on a larger number of patients is warranted.
Introduction. At the very beginning of the Corona virus epidemic there was not enough data on whether hemodialysis patients have a higher risk for Corona virus infection and which factors may affect the severity of clinical picture. Objective. The aim of the study was to determine the significance of the Modified Early Warning Assessment (MEWS) score for the assessment of coronavirus disease exacerbation. Methods. The research was conducted in COVID dialysis, as a retrospective, descriptive-analytical study, at the University Clinical Center Kragujevac, Serbia, which was organized ad-hoc for treatment of SARS-Cov-2 infection positive patients, which are transfered from Center for Hemodialysis "Ćuprija". They were evaluated routine laboratory findings, demographic and gender structure, arterial blood pressure, presence of comorbidities and residual diuresis, duration of dialysis, radiological evaluation of lungs, determination of MEWS score were the parameters that were monitored. The results were monitored on admission and and in the end of treatment. Results. A statistically significant difference was registered in serum lactate dehydrogenase concentration (486 ± 107.62 vs. 423.7 ± 92.4 U/L); p = 0.022 and absolute monocyte count (0.46 ± 0.15 vs. 0.67 ± 0.34 x 103; p = 0.008). The significant increase in MEWS score was also found (b = 0.017; p = 0.030). There was a positive correlation between increase of MEWS score and age (b = 0.027; p = 0.002) and arterial hypertension as a concomitant comorbidity (b = 0.700; p = 0.033). Conclusion. In the observed period, there was a significant increase in the degree of MEWS score of dialysis patients who had SARS-Cov-2 infection.
