A new algorithm has been developed for delineation of significant points of various electrocardiographic signal (ECG) waves, taking into account information from all available leads and providing similar or higher accuracy in comparison with other modern technologies. The test results for the QT database show a sensitivity above 97% when detecting ECG wave peaks and 96% for their onsets and offsets, as well as better positive predictive value compared to the previously known algorithms. In contrast to the previously published algorithms, the proposed approach also allows one to determine the morphology of waves. The segmentation mean errors of all significant points are below the tolerances defined by the Committee of General Standards for Electrocardiography (CSE).
Цель: разработка эффективной стратегии терапии больных психотическими расстройствами вследствие употребления синтетических каннабиноидов. Материал: 43 больных от 18 до 45 лет в связи с употреблением СК и развитием психотического состояния в 2014-2016 г. проходили стационарное лечение. Методы: клинико-анамнестический, клиникопсихопатологический, клинико-катамнестический, лабораторно-инструментальный, статистический. Всем проведено химико-токсикологическое исследование проб мочи. Результаты: сравнительное изучение терапевтической эффективности различных методов лечения психозов вследствие употребления синтетических каннабиноидов показало, что включение в комплексную терапию психозов нейролептических средств повышает эффективность терапии, улучшает приверженность больных к лечению и увеличивает вероятность отказа больных от возобновления приема синтетических каннабиноидов. Currently, a number of manufacturers offer devices for self-registration of ECG and other parameters of the cardiovascular system (CVS) by patients and signal analysis using telemedicine technologies. This makes it possible to create medical services based on mobile remote monitoring. The purpose of the article: a review of existing telemedicine solutions for individual ECG recording and related mobile applications and server-side data analysis software for assessing applicability in functional diagnostics and cardiology services. The article highlights the history of the development of methods for telemedicine analysis of ECG, provides a comparative review of modern solutions for medical cardioregistration. Findings. 1. Individual ECG telemonitoring is a promising technology that is comparable in terms of diagnostic capabilities to assess cardiac rhythm disturbances with Holter ECG monitoring and multifunctional monitoring implanted with ECG loopback recorders. The main vector of development of individual ECG telemonitoring systems is related to the automation of ECG analysis both on the server side and in the patient’s mobile application, for this the application of artificial intelligence and big data (bigdata) is promising. 2. Telecardiogram of an electrocardiogram promotes closer contact of the patient and medical service at the minimum expenses of time for such interaction.
