In the paper we present results of computational experiments to suggest fhe possibility of a general optimality condition of complex systems: a system demonstrates the optimal performance for a problem, when the structural complexity of the system is in a certain relationship with the structural complexity of theproblem. Treatment of hypervascular formations is challenging surgery. The solution of this problem is due to use a multidisciplinary approach, involving complex endovascular and surgical treatments. Patho-morphological study of remote hypervascular formations in treatment was carried out. That treatment consisted of an endovascular embolotherapy and an endovasal sclerotherapy in various combinations. The embolization of blood vessels leads to a complete reduction of blood flow in the formation area, the development of diffuse inflammatory infiltration in the background on tissue ischemia. In the future, developing ischemic necrosis fol-lowed by scarring of tissue surrounding the vascular formation. Sclerotherapy causes damage to the vascular endothelium with fibrin formation and hemolysis of red blood cells in the lumen of the veins. Moreover, there are a significant fibrosis, hyalinosis and inflammatory reaction of the tissues surrounding the vessels. Further development of this process leads to a final sclerosis and fibrotic scarring of the surrounding tissues. The combi-nation of endovascular embolization and sclerotherapy allows to achieve a reduction of blood flow in the arterial vascular malformation department and to cause a tissue fibrosis in the cavernous malformations. Endovascular therapy reduces the risk of bleeding in surgery and develops the capacity of radical treatment.
Purpose of the study: to study the efficacy and safety of Ranibizumab in patients with pathologic myopia and choroidal neovascularization (CNV).22patients (22eyes) with myopic CNV were included in the study. Mean age was 54.0±14.4years, axial length – 28.25±1.9mm, follow-up – from 12 to 59months (mean follow-up – 26.9±13.8months). Ranibizumab was administered intravitreally according to the label in patients with active CNV confirmed by fluorescein angiography. The treatment was shown to have favorable early and long-term outcomes. For example, visual acuity increased (from 0.25 to 0.54; p<0.01), central retinal thickness decreased (from 335.8 to 273.25μm; p<0,05), subretinal neovascular membrane area decreased (from 1272 to 969μm; p<0.05). Clinically significant treatment complications were not observed.Conclusions: The study confirmed high efficacy and safety of anti-angiogenic therapy.
The purpose of research is to study in practice recurrent inflammation of the pelvic organs in women as the mainly cause of massive adhesion process and as a result to infertility. The current course of chronic gonococcal infection in women is asymptomatic without clinical manifestations. During 2009-2013 observation, examination and treatment 198 women were carried out. Women had a history of previous inflammatory diseases of the small pelvis, the diplococcic was revealed to these women during examination. Earlier repeatedly postponed inflammatory diseases of the small pelvis were in history of the women, as well as the patients after laparoscopic operations for infertility. All women previously had regular courses of conservative anti-inflammatory therapy. tively, they didn´t care for nothing. Periodically during preventive examinations or after scheduled examinations gynecologist, the increased number of leukocytes was noted in smears on the flora, in smears on oncocytology – the cytogram of inflammation. Neisseria gonorrhoea was revealed in most cases using the scraping of the three points by PCR. Neisseria gonorrhoeae has been combined : Escherichia coli (massive or modest growth), Trichomonas vaginalis, Enterococcus faecalis (massive or modest growth), U. Urealyticum (or M. Genitalium), Staphylococcus aureus (massive or moderate growth). The prescription of the Metronidazole in the dose of 0,5 mg 3 times per day (every 8 hours) for 2-3 days prior to the appointment of antibacterial drugs has brought the best result. Selection of antibiotics was reduced antibiotic therapy 3-4-5 generations: penicillins, cephalosporins ,carbapenems and monobactams, having (according to bacteriological examination) increased sensitivity to these strains of microorganisms. Conclusions: The patients with recurrent chronic inflammatory diseases of the pelvic organs in women scraping three-point PCR method shows better results identify Neisseria gonorrhoeae.
