Mediastinitis as complications of traumas and esophagus perforations are ill-controlled and characterized with high mortality. Yet clinical characteristics and therapeutic controllability of contact odontogenic mediastinitis are not covered enough in medical literature. Aim of the study was to investigate clinical characteristics and to analyze treatment results of odontogenic purulent mediastinitis according to data of pilot monocenter observational study. Material and methods. The results of treatment of the patients with odontogenic mediastinitis were analyzed in groups: panmediastinitis (8 participants) and posterior upper mediastinitis (12 participants). The following indexes were compared: body temperature, content of C-reactive protein, leucocytes, thrombocytes, creatinine, 24-hour and cumulative fluid electrolyte balance reproduced in hospitals of any equipment levels with using of criteria of nonparametric statistics for excluding of false positive results. Results. Causative agents of the infectious process were found in 65 % of patients in bacteriological study. The mortality was 5 %; the process controllability in conditions of described techniques of anesthesiology and intensive therapy has been shown, the danger of infusive overload of the interstitium of the lungs has been demonstrated.
The aim is to determine the most significant predictors of the oral lichen planus in patients with gastrointestinal pathology.Materials and methods. The study was performed of 40 patients aged 41 to 86 years with oral lichen planus. Degree of mucosal damage by oral lichen planus was evaluated. The concomitant pathology of the gastrointestinal tract was established. A discriminant analysis was carried out.Results. The most significant predictors for a diagnosis of oral lichen planus in patients with gastrointestinal pathology are revealed. The sensitivity of the method was 100.0%, the specificity was 100.0%, and the errorfreeness was 100.0%.The conclusion. Discriminant analysis allows us to conduct a diagnosis of the oral lichen planus in patients with gastrointestinal pathology. This model is available in clinical practice.
Methods of light, electronic microscopy and immunohystochemistry lead research of structural changes of a mucous membrane maxillary sinus at 43 patients with odontogenic perforative sinusitis. The material is broken into 4 groups depending on a limitation period of punching. Degenerate changes ciliated epithelial cells came to light already on early terms after perforations of sinusitis and were shown by reduction of specific volume of mitochondrias, the GER, increase NCR. In globlet exocrine cells reduction NCR is connected to infringement of evacuation of a secret. In terms more than 3 months occured metaplasia of epithelium in single-row cubic and multilayered flat with keratosis, increased cellular infiltration own plate of a mucous membrane maxillary sinus. The contents of CD4 + cells in a mucous membrane of sine gradually increased, and on late terms of occurrence of perforations decreased. The contents of CD8+ and СD20+ cells in a mucous membrane with perforations of sinusitis were raised.

Research objective was establishment of ultrastructural changes of mucous membrane maxillary sinus (MM MS) at noninvasive and chronic invasive aspergillosis. By methods of light, electronic microscopy and morphometry it is established, that the basic signs of endocellular reorganisation of epithelial cells at noninvasive form are decrease in squirrels-synthetic and plastic functions. On separate sites of epithelium there are the cages characterised small changes by cytoplasmatic organells. As a whole, the revealed changes testify to loss of epithelium protective and barrier properties, and as consequence, its defective participation in mucocilliar clearance. At invasive form of aspergillosis sinusitis, despite revealed single-layered metaplasia, in epithelial cells was observed fiber-synthetic and plastic processes that can specify on high reparative possibilities of epithelium.
