В статье представлено современное состояние психиатрической помощи в мире, приведены результаты деинституционализации психиатрической помощи в разных странах, проанализированы основные направления реформы психоневрологических интернатов в Российской Федерации. The article reviews the current state of psychiatric care in the world, presents the results of the deinstitutionalization of psychiatric care in different countries, and analyzes the main directions of the reform of neuropsychiatric boarding institutions in the Russian Federation.

В последнее время наблюдается рост фармакорезистентных психических расстройств. В таких случаях доказала свою эффективность и безопасность модифицированная электросудорожная терапия (ЭСТ). В ГУ «Республиканский научно-практический центр психического здоровья» (Минск) метод внедрен в клиническую практику и проводятся научные исследования по персонализации режимов выполнения процедуры ЭСТ. Разработаны инструкции по применению ЭСТ в условиях стационара, в амбулаторных условиях, комбинированному применению с транскраниальной магнитной стимуляцией. Recently, there has been an increase in drug-resistant mental disorders. In such cases, modified electroconvulsive therapy has proven to be effective and safe. In the Republican Scientific and Practical Center for Mental Health, the method has been introduced into clinical practice and scientific research is being carried out to personalize the modes of performing the ECT procedure. Instructions have been developed for the use of ECT in a hospital setting, in an outpatient setting, and for combined use with transcranial magnetic stimulation.
